子午流注择时穴位按摩联合中药熏香对血液透析患者睡眠质量和疲乏的影响

Influence of meridian flow injection timing acupoint massage combined with herbal incense on sleep quality and fatigue in hemodialysis patients

目的探究子午流注择时穴位按摩联合中药熏香对血液透析患者睡眠质量和疲乏的影响。方法回顾性选取2022年4月至2024年4月在丽水市中医院肾内科接受规律性血液透析治疗的94例尿毒症患者进行分析,依据治疗方案差异分为联合组(n=33)、中药熏香组(n=32)与对照组(n=29)。对照组予以常规治疗,中药熏香组基于对照组联合中药熏香治疗,联合组基于中药熏香组联用子午流注择时穴位按摩治疗。比较三组疗效、不良反应、治疗前后的疲乏程度及睡眠状况评分修订版Piper疲乏量表(RPFS)、匹茨堡睡眠质量指数(PSQI):、客观睡眠指标(总睡眠时间、睡眠潜伏期、觉醒时间)以及炎症指标肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、C反应蛋白(CRP):。结果疗效比较,联合组>中药熏香组>对照组(P<0.05);三组治疗后的RPFS、PSQI评分较治疗前均降低,且联合组<中药熏香组<对照组(P<0.05);三组治疗后的总睡眠时间、睡眠潜伏期、觉醒时间较治疗前均改善,且联合组改善更优(P<0.05);三组治疗后的CRP、TNF-α、IL-1β较治疗前均降低,且联合组<中药熏香组<对照组(P<0.05);不良反应情况比较,三组无显著差异(P>0.05)。结论子午流注择时穴位按摩联合中药熏香可显著提升血液透析患者睡眠质量,改善疲乏症状,且具备较好安全性。

Objective To explore the effects of meridian flow injection timing acupoint massage combined with herbal incense on sleep quality and fatigue in hemodialysis patients.Methods Ninety-four patients with uremia who received regular hemodialysis treatment in the Department of Nephrology,Lishui Hospital of Traditional Chinese Medicine from April 2022 to April 2024 were retrospectively selected for analysis.According to the differences in treatment plans,they were divided into combination group(n=33),herbal incense group(n=32)and control group(n=29).The control group was given conventional treatment,the herbal group was based on the control group combined with herbal incense,and the combination group was based on the herbal incense group combined with meridian flow injection timing acupoint massage.The efficacy,adverse reactions,fatigue and sleep status scores before and after treatment(Revised Piper Fatigue Scale(RPFS),Pittsburgh Sleep Quality Index(PSQI)),objective sleep indicators(total sleep time,sleep latency,and wake time),and inflammatory indicators tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),and C-reactive protein(CRP):were compared among the three groups.Results The comparison of curative effects showed that combination group>herbal incense group>control group(P<0.05).The RPFS and PSQI scores of the three groups after treatment were decreased compared with those before treatment,and the combination group was<herbal incense group<control group(P<0.05).The total sleep time,sleep latency and wake time of the three groups after treatment were improved compared with those before treatment,and the improvement was better in the combination group(P<0.05).The levels of CRP,TNF-αand IL-1βin the three groups after treatment were decreased compared with those before treatment,and the combination group was<herbal incense group<control group(P<0.05).There was no significant difference in adverse reactions among the three groups(P>0.05).Conclusion Meridian flow injection timing acupoint massage combined with herbal incense can significantly improve the sleep quality of hemodialysis patients,improve fatigue symptoms,and have good safety.
