妇科术后病人早期活动最佳证据的临床实践

Clinical practice of best evidence of early activity in gynecological postoperative patients

目的:将妇科术后病人早期活动的最佳证据应用于临床实践,并评价其应用效果,为妇科术后病人早期活动实践提供可靠的循证医学证据支持。方法:选取某三级甲等妇产科医院2021年7月—10月住院的手术病人为研究对象。将2021年7月—8月的100例病人作为对照组;将2021年9月—10月的100例病人作为干预组。在循证实践的基础上进行妇科术后病人早期活动的最佳证据应用,观察证据应用前后病人的结局指标、护理人员对妇科术后早期活动的知识水平及审查指标的达标情况。结果:最佳证据的应用缩短了病人术后首次排气和住院时间,提高了病人满意度,差异均有统计学意义(P<0.05);护理人员知识考核合格率从15.79%提高至84.21%,差异有统计学意义(P<0.05);审查指标中的指标3、6、7、8和10的执行率均从0提升至100%,指标2、4、11和12的执行率分别从92%、87%、26%、0提升至99%、96%、99%、99%,差异均有统计学意义(P<0.05)。结论:妇科术后早期活动最佳证据的应用缩短了病人首次排气时间和术后住院时间,提高了病人的满意度及护理人员的知识水平。

Objective:To apply the best evidence of early activity of gynecological postoperative patients into clinical practice and evaluate its application effect,so as to provide reliable evidence-based medical evidence support for the practice of early activity of gynecological postoperative patients.Methods:The surgical patients hospitalized from July to October 2021 in a tertiary class A obstetrics and gynecology hospital were selected as the study objects.100 patients from July to August 2021 will be used as control group;100 patients from September to October 2021 were selected as the intervention group.On the basis of evidence-based practical,the best evidence of early activity of patients after gynecological surgery was applied to observe the outcome indicators of patients before and after the application of evidence,the knowledge level of nurses on early gynecological activity after surgery and the compliance of review indicators.Results:The application of the best evidence shortened the patient's first postoperative exhaust and hospital stay,and improved the patient's satisfaction,the difference was statistically significant;The qualified rate of knowledge assessment of nursing staff increased from 15.79%to 84.21%,and the difference was statistically significant(P<0.05).Among the review indicators,the implementation rates of indicators 3,6,7,8 and 10 were all increased from 0 to 100%,and the implementation rates of indicators 2,4,11 and 12 were increased from 92%,87%,26%and 0 to 99%,96%,99%and 99%,respectively,with statistically significant differences(P<0.05).Conclusion:The application of the best evidence of early activity after gynecological surgery can shorten the time of first exhaust and postoperative hospital stay,improve the satisfaction of patients and the knowledge level of nurses.
