基于DRG的甲状腺癌KD19病组手术患者住院费用分析

DRG-Based Analysis of Changes in Hospitalization Expenses for Patients with Thyroid Cancer in KD19 Disease Group

目的分析甲状腺KD19病组患者住院费用变化情况,为合理控制住院费用和优化费用结构提供参考。方法选取2019—2023年出院主诊断为甲状腺癌且DRG病组为KD19患者的病案首页,使用灰色关联度和结构变动分析法分析住院费用的影响因素及其变动情况。结果共纳入3921例KD19病组患者,五年间该病组患者平均住院日、次均住院费用呈下降趋势。耗材费用与住院费用的关联度最强。2019—2023年药品费用结构变动值为负值,医疗、耗材费用均为正值,三者结构变动贡献率累计达88.13%。结论5年间KD19病组患者服务效率提升,住院费用有所控制,费用结构得以优化。但住院费用,尤其是耗材费用需加强控制,费用结构优化仍有提升空间。

Objective To analyze the changes of hospitalization expenses in patients with thyroid cancer in KD19 disease group,and to provide reference for reasonable control of hospitalization expenses and optimization of cost structure.Methods The first page of medical records of patients discharged from 2019 to 2023 with the main diagnosis of thyroid cancer and the DRG group as KD19 were selected.The grey correlation analysis and structure change analysis were used to analyze the influencing factors and changes of hospitalization expenses.Results A total of 3921 patients with KD19 disease were enrolled in this study.The average length of stay and the average hospitalization expenses showed a downward trend during the five years.Consumables costs had the strongest correlation with hospitalization costs.From 2019 to 2023,the change value of the drug cost structure was negative,and the medical service and consumables costs were both positive,with the cumulative contribution rate of the three structural changes reaching 88.13%.Conclusion In the past five years,the service efficiency of KD19 patients has been improved,the hospitalization cost has been controlled,and the cost structure has been optimized.However,hospitalization costs,especially the costs of consumables,need to be strengthened,and there is still room for improvement in the optimization of cost structure.
