Effects of sexual health education on sexual rehabilitation in breast cancer patients:a Meta-analysis

目的系统评价性健康教育对乳腺癌患者性生活质量、性生活满意度、心理状况和生活质量的影响。方法系统检索Cochrane Library、Embase、PubMed、中国知网、万方数据库、维普网和中国生物医学文献数据库中关于性健康教育对乳腺癌患者性生活质量、性生活满意度、焦虑、抑郁和生活质量影响的临床随机对照试验(RCT),检索时限均为建库至2023年11月8日。2名研究者独立筛选文献、提取数据,并使用Cochrane手册5.1.0推荐的RCT偏倚风险评估工具评价文献质量。采用RevMan 5.4和R4.3.2软件进行Meta分析。结果共纳入17篇文献,共1695例患者。性健康教育在改善乳腺癌患者的性生活质量(SMD=1.25,95%CI:0.65~1.84,P<0.05)、性生活满意度(SMD=0.85,95%CI:0.53~1.17,P<0.05)、抑郁(SMD=-1.16,95%CI:-2.01~-0.30,P<0.05)和生活质量(SMD=1.53,95%CI:0.09~2.97,P<0.05)方面均有统计学意义,对焦虑的改善效果不具有统计学意义(SMD=-0.88,95%CI:-2.06~0.29,P=0.14)。结论性健康教育能改善乳腺癌患者性生活质量、性生活满意度、抑郁和生活质量,而对焦虑状况的改善效果不明显,可能与该结局指标纳入文献数量较少有关,未来可设计RCT探讨性健康教育对焦虑的影响。

ObjectiveTo systematically evaluate the impact of sexual health education on the sexual quality of life,sexual satisfaction,anxiety,depression and the quality of life in breast cancer patients.MethodsA comprehensive search was conducted in Cochrane Library,Embase,PubMed,CNKI,Wanfang Data,VIP,and SinoMed databases for randomized controlled trials(RCTs)exploring the effects of sexual health education on sexual quality of life,sexual satisfaction,anxiety,depression and quality of life in breast cancer patients.The search covered studies up until November 8,2023.Two researchers independently screened the literature,extracted data,and assessed the quality of the RCTs using the Cochrane Handbook 5.1.0 risk of bias tool.Meta-analysis was performed using RevMan 5.4 and R4.3.2 software.ResultsA total of 17 articles with 1695 patients were included.Sexual health education significantly improved sexual quality of life[SMD=1.25,95%CI(0.65,1.84),P<0.05],sexual satisfaction[SMD=0.85,95%CI(0.53,1.17),P<0.05],depression[SMD=-1.16,95%CI(-2.01,-0.30),P<0.05],and the quality of life[SMD=1.53,95%CI(0.09,2.97),P<0.05]in breast cancer patients.However,the improvement in anxiety was not statistically significant[SMD=-0.88,95%CI(-2.06,0.29),P<0.05].ConclusionsSexual health education can improve the sexual quality of life,sexual satisfaction,depression,and the quality of life in breast cancer patients.However,its impact on anxiety remains unclear,possibly due to the limited number of studies.Future RCTs should further explore the effect of sexual health education on anxiety in this population.
