甘精胰岛素联合阿卡波糖片在2型糖尿病患者中的临床疗效、安全性观察

Clinical efficacy and safety of Insulin Glargine combined with Acarbose Tablets in patients with type 2 diabetes mellitus

目的探讨甘精胰岛素联合阿卡波糖片在2型糖尿病患者中的临床疗效、安全性观察。方法选取2018年1月至2023年8月天水市中医医院内分泌科门诊收治的60例2型糖尿病患者作为研究对象,按照随机数字表法分为观察组(30例)与对照组(30例),对照组采用甘精胰岛素治疗方法,观察组采用甘精胰岛素联合阿卡波糖片治疗方法,比较两组的临床疗效、血糖及胰岛功能指标、血糖波动及相关指标、安全性。结果观察组治疗总有效率为96.67%,高于对照组的70.00%,差异有统计学意义(P<0.05);观察组治疗后空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)低于对照组,胰岛β细胞功能指数(HOMA-β)高于对照组,差异有统计学意义(P<0.05);观察组治疗后胰岛素生长因子-1(IGF-1)、血糖波动、夜间低血糖发生率低于对照组,血糖达标时间短于对照组,差异有统计学意义(P<0.05);两组患者头晕、恶心呕吐、腹胀、肠鸣音亢进、心慌、冷汗等不良反应总发生率比较,差异无统计学意义(P>0.05)。结论甘精胰岛素联合阿卡波糖片在2型糖尿病患者中的应用效果显著,能提高血糖控制效果,减少血糖波动,保护胰岛功能,缩短血糖达标时间,减少夜间低血糖发生率。

Objective To investigate the clinical efficacy and safety of Insulin Glargine and Acarbose Tablets in patients with type 2 diabetes.Methods Sixty patients with type 2 diabetes admitted to the Department of Endocrinology,Tianshui Hospital of Traditional Chinese Medicine from January 2018 to August 2023 were selected as the study objects,and they were divided into observation group(30 cases)and control group(30 cases)according to random number table method.The control group was treated with Insulin Glargine,and the observation group was treated with Insulin Glargine combined with Acarbose Tablets.The clinical efficacy,blood glucose and islet function indexes,blood glucose fluctuation and related indexes,and safety of the two groups were compared.Results The total effective rate of the observation group was 96.67%,which was higher than that of the control group(70.00%),and the difference was statistically significant(P<0.05).After treatment,fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c)and insulin resistance index(HOMA-IR)in observation group were lower than those in control group,and islet beta cell function index(HOMA-β)was higher than those in control group,with statistical significances(P<0.05).After treatment,the incidence of insulin growth factor-1(IGF-1),blood glucose fluctuation and night hypoglycemia in observation group was lower than that in control group,and the blood glucose compliance time was shorter than that in control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions such as dizziness,nausea and vomiting,abdominal distension,hyperbowel sound,palpitation and cold sweat between two groups(P>0.05).Conclusion The application of insulin glargine combined with acarbose tablets in patients with type 2 diabetes mellitus has significant effects,which can improve blood glucose control,reduce blood glucose fluctuation,protect islet function,shorten the time of blood glucose compliance,and reduce the incidence of nocturnal hypoglycemia.
