清热滋阴汤配合穴位贴敷治疗痰热伤阴型慢性阻塞性肺疾病急性加重期的临床观察

Clinical observation of Qingre Ziyin Decotion combined with acupoint application in the treatment of acute exacerbation of chronic obstructive pulmonary disease of phlegm heat injuring yin type

目的探讨清热滋阴汤配合穴位贴敷治疗痰热伤阴型慢性阻塞性肺疾病急性加重期(AECOPD)的临床效果。方法选取2023年5月至2024年3月抚州市中医医院收治的60例AECOPD患者作为研究对象,按照随机数字表法分为对照组(30例)和观察组(30例)。对照组采用常规西医治疗,观察组加用清热滋阴汤配合穴位贴敷治疗,两组均连续治疗10 d。比较两组的临床疗效、症状和体征积分、肺功能、6分钟步行距离(6MWT)及实验室指标。结果观察组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05);两组患者治疗后症状、体征积分及白细胞(WBC)计数、中性粒细胞百分比(N%)、嗜酸性粒细胞百分比(EOS%)、血清肿瘤坏死因子-α(TNF-α)水平低于本组治疗前,且观察组指标低于对照组,差异有统计学意义(P<0.05);两组患者第1秒用力呼气容积占预计值百分比(FEV_(1)%)、用力肺活量占预计值百分比(FVC%)、6分钟步行距离(6MWT)高于本组治疗前,且观察组指标高于对照组,差异有统计学意义(P<0.05)。结论清热滋阴汤配合穴位贴敷治疗痰热伤阴型AECOPD患者安全有效,利于控制炎症反应,改善肺功能,值得临床推广应用。

Objective To explore the clinical effect of Qingre Ziyin Decotion combined with acupoint application in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients with phlegm heat injuring yin type.Methods A total of sixty patients with AECOPD admitted to Fuzhou Hospital of Traditional Chinese Medicine from May 2023 to March 2024 were selected as the research subjects and they were divided into control group(30 cases)and observation group(30 cases)according to random number table method.The control group received conventional western medicine treatment,while the observation group received Qingre Ziyin Decotion combined with acupoint application.Both groups were treated continuously for 10 days.The clinical efficacy,symptom and sign scores,lung function,6-minute walking distance(6MWT),and laboratory indicators were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the symptom and sign scores,white blood cell(WBC),neutrophil percentage(N%),percentage of eosinophils(EOS%),serum tumor necrosis factor-α(TNF-α)levels in both groups were lower than those before treatment,and the indexes of observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The forced expiratory volume in one second(FEV_(1)%),forced vital capacity(FVC%),and 6-minute walking test(6MWT)of two groups were higher than those before treatment,and the indexes of observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Qingre Ziyin Decotion combined with acupoint application is safe and effective for the treatment of AECOPD patients with phlegm heat injuring yin type,which is beneficial for controlling inflammatory reactions and improving lung function.It is worthy of clinical application.
