亮丙瑞林联合左炔诺孕酮宫内缓释系统在异常子宫出血患者中的应用效果

Application effect of Leuprorelin combined with levonorgestrel intrauterine sustained-release system in patients with abnormal uterine bleeding

目的探讨异常子宫出血(AUB)患者采用亮丙瑞林联合左炔诺孕酮宫内缓释系统(LNG-IUS)治疗效果。方法选取2022年1月至2023年10月江西省上饶市人民医院的72例AUB患者作为研究对象,采用随机数字表法分为对照组和观察组,每组各36例。对照组采用LNG-IUS治疗,观察组于对照组基础上采用亮丙瑞林治疗。比较两组临床疗效、完全止血时间、子宫内膜厚度(EmT)、月经量、性激素[黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)]及不良反应。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组治疗后的完全止血时间短于对照组,EmT、月经失血图(PBAC)评分及LH、FSH、E2水平低于对照组,差异有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论AUB患者采用亮丙瑞林与LNG-IUS联合治疗能够调节LH、FSH及E2水平及月经情况,改善EmT,缩短完全止血时间,且不良反应并未增加。

Objective To evaluate the efficacy of Leuprolide combined with levonorgestrel intrauterine sustained release system(LNG-IUS)in the treatment of abnormal uterine bleeding(AUB).Methods A total of 72 patients with AUB from Shangrao People's Hospital of Jiangxi Province from January 2022 to October 2022 were selected as the study objects,and they were divided into control group and observation group by random number table method,with 36 patients in each group.The control group was treated with LNG-IUS,the observation group was treated with Leuprolide on the basic of the control group.The clinical efficacy,complete hemostasis time,endometrial thickness(EmT),menstrual volume,luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E2)and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The time of complete hemostasis after treatment in the observation group was shorter than that in the control group,and the scores of EmT,pictorial blood loss assessment chart(PBAC)and LH,FSH and E2 levels were lower than those in the control group,with statistical significance(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Leuprolide combined with LNG-IUS can regulate the levels of LH,FSH,E2 and menstruation,improve the thickness of endometrium and shorten the time of complete hemostasis in patients with AUB.
