鼓室内注射甲泼尼龙琥珀酸钠联合银杏叶提取物治疗突发性聋的效果

Effect of intratympanic injection of Methylprednisolone Sodium Succinate combined with Ginkgo Biloba Extract in treatment of sudden hearing loss

目的探讨鼓室内注射甲泼尼龙琥珀酸钠联合银杏叶提取物治疗突发性聋(SHL)的效果。方法回顾性分析2020年12月至2023年12月厦门医学院附属第二医院收治的60例SHL患者的临床资料,根据患者治疗方案的差异,分为JP组(30例)和JP+YX组(30例)。JP组行鼓室内注射甲泼尼龙琥珀酸钠,JP+YX组在JP组基础上应用银杏叶提取物,比较两组治疗总有效率、听力水平、血液流变学指标、中医证候积分、不良反应总发生率。结果JP+YX组临床总有效率高于JP组,差异有统计学意义(P<0.05)。治疗前,JP+YX组、JP组听力值比较,差异无统计学意义(P>0.05);治疗后,JP+YX组、JP组听力值低于本组治疗前,且JP+YX组听力值低于JP组,差异有统计学意义(P<0.05)。治疗前,JP+YX组、JP组血液流变学指标值比较,差异无统计学意义(P>0.05);治疗后,JP+YX组、JP组血液流变学指标值低于本组治疗前,且JP+YX组血液流变学指标值低于JP组,差异有统计学意义(P<0.05)。治疗前,JP+YX组、JP组耳鸣、耳聋、耳胀闷、头痛各项中医证候积分比较,差异无统计学意义(P>0.05);治疗后,JP+YX组、JP组中医证候积分低于本组治疗前,且JP+YX组中医候积分低于JP组,差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论SHL患者应用鼓室内注射甲泼尼龙琥珀酸钠联合银杏叶提取物治疗效果良好,既能提高听力水平,还可改善中医证候及血液流变学状态,安全性良好,值得推广。

Objective To explore the effect of intratympanic injection of Methylprednisolone Sodium Succinate combined with Ginkgo Biloba Extract in the treatment of sudden hearing loss(SHL).Methods The clinical data of 60 patients with SHL admitted to the Second Affiliated Hospital of Xiamen Medical College from December 2020 to December 2023 were retrospectively analyzed and they were divided into JP group(30 cases)and JP+YX group(30 cases)according to the differences in treatment plans.JP group was injected with Methylprednone Sodium Succinate in tympanic chamber,and JP+YX group was added with Ginkgo Biloba Extract on the basis of JP group.The total effective rate,hearing level,hemorheology index,traditional Chinese medicine syndrome score and total incidence of adverse reactions were compared between the two groups.Results The total clinical effective rate of JP+YX group was higher than that of JP group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in hearing value between JP+YX group and JP group(P>0.05).After treatment,the hearing values of JP+YX group and JP group were lower than those before treatment,and hearing value of JP+YX group was lower than that of JP group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in hemorheological indexes between JP+YX group and JP group(P>0.05).After treatment,the hemorheological index values of JP+YX group and JP group were lower than those before treatment,and the hemorheological index values of JP+YX group were lower than those of JP group,the differences were statistically significant(P<0.05).Before treatment,there were no statistical significance in the scores of tinnitus,deafness,ear swelling and headache between JP+YX group and JP group(P>0.05).After treatment,the scores of traditional Chinese medicine symptoms in JP+YX group and JP group were lower than those before treatment,and the scores of traditional Chinese medicine symptoms in JP+YX group was lower than JP group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Tympanic injection of Methylprednisolone Sodium Succinate combined with Ginkgo Biloba Extract for SHL patients has good therapeutic effect,which can not only improve hearing level,but also improve traditional Chinese medicine syndrome and hemorheology status,and is safe and worthy of promotion.
