米林高寒高海拔地区人群膝骨关节炎超声特征及相关影响因素筛选

Ultrasound characteristics of knee osteoarthritis and related influencing factors in a population in the alpine and high-altitude region of Milin

目的研究超声诊断米林高寒高海拔地区膝骨关节炎(KOA)的价值及筛选该病的相关影响因素。方法2022年3-5月对米林县内膝关节不适人群进行临床及超声检查。结果受试者共608人,男200人,女408人;平均年龄(57.86±11.36)岁,病程7 d~42年,中位病程4年。纳入膝关节1216个,其中确诊971个,男性31.62%,女性68.38%;左侧486人,右侧485人,双侧478人。各年龄组总体组间差异有统计学意义。超声诊断KOA的敏感度为0.93,特异度为0.36,阳性预测值为0.85,阴性预测值为0.57。姿势及饮酒分别与有无KOA组间差异有统计学意义;BMI、吸烟和饮食与KOA组间差异均无统计学意义。结论在米林高寒高海拔地区,超声对膝骨关节炎具有较高敏感度;姿势及饮酒与罹患该病相关,BMI、吸烟及饮食则无明确关联。

Objective To study the value of ultrasound in diagnosing knee osteoarthritis(KOA)in the alpine and high-altitude areas of Milin and screen its relevant influencing factors.Methods Clinical and ultrasonographic examinations were performed from March to May 2022 on people with knee discomfort within Milin County.Results A total of 608 subjects were enrolled,including 200 males and 408 females;their mean age was(57.86±11.36)years,disease duration was 7 days-42 years,and median disease duration was 4 years.A total of 1216 knees were included,of which 971 were diagnosed,including 31.62%male and 68.38%female;of which 486 were left-sided,485 right-sided and 478 bilateral.The differences between age groups were statistically significant.The sensitivity of ultrasound diagnosis of KOA was 0.93,the specificity was 0.36,the positive predictive value was 0.85,and the negative predictive value was 0.57.There were statistically significant differences in postural and alcohol consumption between groups with and without KOA;there were no statistically significant differences in BMI between the KOA groups;and there were no statistically significant differences in smoking and diet between groups with and without KOA.Conclusion Ultrasound is highly sensitive to knee osteoarthritis in the alpine and high altitude region of Milin;posture and alcohol consumption were associated with the development of this disease,while BMI,smoking and diet were not clearly associated.
