摘要
目的:探讨神经肌电图检查在糖尿病周围神经病变诊断中的应用价值。方法:选取2021年1月—2023年12月常州市第二人民医院收治的糖尿病周围神经病变患者150例作为观察组,选取同期收治的单纯糖尿病患者100例作为对照组,选取同期体检健康者100例作为健康组。三组均进行神经肌电图检查。比较三组以及观察组3个亚组[根据病程分为观察1组(病程<1年)、观察2组(病程1~10年)和观察3组(病程>10年)]神经肌电图检查结果。结果:观察组正中神经、尺神经、腓总神经运动神经传导速度(MCV)及感觉神经传导速度(SCV)低于对照组、健康组(P<0.05)。观察组3个亚组正中神经、尺神经、腓总神经MCV、复合肌肉动作电位波幅、SCV、感觉神经动作电位波幅对比,观察3组<观察2组<观察1组,差异有统计学意义(P<0.05);观察组3个亚组正中神经、尺神经、腓总神经远端运动潜伏期,MCV、SCV、F波和H反射异常率对比,观察组3组>观察2组>观察1组,差异有统计学意义(P<0.05)。结论:神经肌电图检查在糖尿病周围神经病变诊断中的应用价值较高,随着患者病变程度加重,其神经电传导速度逐渐变慢。
Objective:To explore the application value of neuroelectromyography in diagnosis of diabetic peripheral neuropathy.Methods:A total of 150 patients with diabetic peripheral neuropathy admitted to Changzhou No.2 People's Hospital from January 2021 to December 2023 were selected as the observation group.A total of 100 patients with diabetes alone admitted during the same period were selected as the control group.A total of 100 healthy people undergoing physical examination during the same period were selected in the healthy group.Neuroelectromyography was performed in all three groups.The neuroelectromyography results were compared among three groups and 3 subgroups in the observation group[According to the course of the disease,the observation group was divided into observation group 1(disease course of<1 year),observation group 2(disease course of 1-10 years)and observation group 3(disease course of>10 years)].Results:The motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of the median nerve,ulnar nerve,common peroneal nerve,in the observation group were lower than those in the control group and the healthy group(P<0.05).The median nerve,ulnar nerve and common peroneal nerve MCV,compound muscle action potential,SCV and sensory nerve action potential among 3 subgroups in the observation group were compared and ranked as follows:observation group 3<observation group 2<observation group 1,and the difference was statistically significant(P<0.05).Observe the latency of distal motor activity of the median nerve,ulnar nerve,and common peroneal nerve in three subgroups of the observation group,MCV,SCV,F wave and H abnormal reflex rates among 3 subgroups in the observation group were compared and ranked as follows:observation group 3>observation group 2>observation group 1,and the difference was statistically significant(P<0.05).Conclusion:Neuroelectromyography has a high application value in the diagnosis of diabetic peripheral neuropathy.As the severity of the lesion increases,the nerve conduction velocity gradually slows.
