精细化护理在急性脑梗死患者机械介入取栓术联合阿替普酶静脉溶栓治疗中的应用效果

Application Effect of Refined Nursing in Mechanical Interventional Thrombectomy Combined with Alteplase Intravenous Thrombolysis Treatment of Acute Cerebral Infarction Patients

目的:探讨精细化护理在急性脑梗死患者机械介入取栓术联合阿替普酶静脉溶栓治疗中的应用效果。方法:选取2022年1月—2023年1月在临沭县人民医院行机械介入取栓术联合阿替普酶静脉溶栓治疗的急性脑梗死患者60例作为观察对象,随机分为对照组(实施常规护理)和观察组(实施精细化护理),各30例。比较两组护理效果。结果:与对照组相比,观察组护理总满意度更高(P<0.05)。护理前,两组美国国立卫生研究院卒中量表(NIHSS)、Barthel指数、简易智力状态检查量表(MMSE)评分比较,差异无统计学意义(P>0.05);护理后,两组NHISS评分降低,Barthel指数、MMSE评分升高,且观察组改善幅度大于对照组(P<0.05)。护理后,两组生理功能、心理功能、物质生活、社会功能及总体健康评分升高,且观察组高于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论:精细化护理在急性脑梗死患者机械介入取栓术联合阿替普酶静脉溶栓治疗中的应用效果显著,可改善患者神经功能、日常生活能力、认知功能,减少并发症,提高护理满意度。

Objective:To explore the application effect of refined nursing in the treatment of acute cerebral infarction patients by mechanical interventional thrombectomy combined with intravenous thrombolysis with alteplase.Methods:A total of 60 patients with acute cerebral infarction who underwent mechanical interventional thrombolysis combined with intravenous thrombolysis with alteplase in Linshu County People's Hospital from January 2022 to January 2023 were selected as the observation objects.They were randomly divided into the control group(routine nursing)and the observation group(refined nursing).The nursing effects were compared between two groups.Results:Compared with the control group,the total satisfaction with nursing in the observation group was higher(P<0.05).Before nursing,there was no significant difference between the two groups in the scores of National Institutes of Health Stroke Scale(NIHSS),Barthel Index,and Mini-Mental State Examination(MMSE)(P>0.05).After nursing,the NHISS score in the two groups decreased,while the Barthel index and MMSE score in the two groups increased,and the improvement in the observation group was greater than that in the control group(P<0.05).After nursing,the scores of physiological function,psychological function,material life,social function and overall health in the two groups increased,and the scores in the observation group were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The application of refined nursing in the treatment of patients with acute cerebral infarction by mechanical interventional thrombectomy combined with intravenous thrombolysis with alteplase has significant effects,and can improve the neurological function,daily living ability and cognitive function of patients,reduce complications and improve nursing satisfaction.
