苏州某社区医院老年慢性病患者跌倒现状及影响因素分析

Analysis of Current Situation and Influencing Factors of Fall in Elderly Patients with Chronic Diseases in a Community Hospital of Suzhou

目的:调查苏州某社区医院老年慢性病患者跌倒现状,分析其影响因素。方法:选取2022年10月—2023年10月于苏州市元和社区医院全科门诊就诊的老年慢性病患者809例作为调查对象,统计近1年内发生跌倒的人数,并将调查对象分为跌倒组和非跌倒组,比较两组一般资料,分析其跌倒的影响因素。结果:809例老年慢性病患者发生跌倒70例,发生率为8.7%。年龄、运动情况、饮酒情况、慢性肾脏病患病情况、血管紧张素转换酶抑制剂服用情况是老年慢性病患者跌倒的影响因素,差异有统计学意义(P<0.05)。Logistic回归分析显示,年龄大、患有慢性肾脏病是老年慢性病患者跌倒的独立危险因素,服用血管紧张素转换酶抑制剂是老年慢性病患者跌倒的独立保护因素(P<0.05)。结论:年龄大、患慢性肾脏病是老年慢性病患者跌倒的独立危险因素,服用血管紧张素转换酶抑制剂是其跌倒的独立保护因素,临床应加强老年慢性病患者跌倒的防范力度,对于跌倒危险因素进行针对性干预。

Objective:To investigate the current situation of fall in elderly patients with chronic diseases in a community hospital of Suzhou,and analyze the influencing factors.Methods:From October 2022 to October 2023,a total of 809 elderly patients with chronic diseases who visited the General Practice Outpatient Clinic of Yuanhe Community Hospital in Suzhou were selected as the survey subjects.The number of patients who experienced fall within recent 1 year was counted.The survey subjects were divided into a fall group and a non-fall group.General information in two groups were compared.The influencing factors for fall were analyzed.Results:Falls occurred in 70 of the 809 elderly patients with chronic diseases,with an incidence rate of 8.7%.Age,exercise,alcohol consumption,chronic kidney disease prevalence and angiotensin converting enzyme inhibitor administration were the influencing factors for fall in elderly patients with chronic diseases,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that older age and chronic kidney disease were independent risk factors for fall in elderly patients with chronic diseases,and angiotensin converting enzyme inhibitor administration was independent protective factor for fall in elderly patients with chronic diseases(P<0.05).Conclusion:Older age and chronic kidney disease are independent risk factors for fall in elderly patients with chronic diseases,and angiotensin converting enzyme inhibitor administration is independent protective factor for fall in such patients.In clinical setting,it is necessary to strengthen the prevention of fall in elderly patients with chronic diseases,and carry out targeted interventions for fall risk factors.
