超声心动图对原发性高血压患者左室心肌功能的评估价值研究

Study on value of echocardiography in evaluating left ventricular myocardial function of patients with essential hypertension

目的分析超声心动图评估原发性高血压患者的左室心肌功能变化及其与分期的关系。方法选择2022年2月—2023年2月本院收治的80例原发性高血压患者(高血压组)及同期40名健康成年志愿者(对照组)作为研究对象,均行超声心动图检查,对比两组超声心动图参数、左室心肌功能参数,比较不同分期高血压患者的超声心动图参数、左室心肌功能参数差异,并分析相关性。结果高血压组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、室间隔舒张末期厚度(IVSD)、左心室后壁舒张末期厚度(LVPWD)、左心室舒张末期容积(EDV)、左心室收缩末期容积(ESV)、舒张晚期血流速度(A)、左心室质量指数(LVMI)均高于对照组,高血压组二尖瓣口舒张早期血流速度(E)、E/A低于对照组(P<0.05);与对照组比较,高血压组的左心室整体纵向收缩期峰值应变(LVGLS)、左心室整体径向收缩期峰值应变(LVGRS)、左心室整体环向收缩期峰值应变(LVGCS)、左心室整体面积收缩期峰值应变(LVGAS)呈下降趋势(P<0.05),且以上指标均随着高血压分期增加而呈递增/递减趋势(P<0.05);原发性高血压患者左室心肌功能参数(LVEDD、LVESD、E/A、LVGLS、LVGRS、LVGCS、LVGAS)与其分期有相关性(P<0.05)。结论超声心动图在原发性高血压患者的左室心肌功能中有较好的评估效果,且能用于判断和分期的关系。

Objective To analyze the changes in left ventricular myocardial function of patients with essential hypertension based on echocardiography and its relationship with staging.Methods From September 2021 to October 2023,eighty patients with essential hypertension admitted to our hospital(hypertension group)and 40 healthy adult volunteers(control group)were included in the study.Echocardiography was performed on all participants.Echocardiography parameters and left ventricular myocardial function parameters were compared between the two groups,and between patients with different stages of hypertension.Correlation analysis was performed.Results The left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),interventricular septum diameter(IVSD),left ventricular posterior end-diastolic thickness(LVPWD),left ventricular end diastolic volume(EDV),left ventricular end systolic volume(ESV),late diastolic blood flow velocity(A),and left ventricular mass index(LVMI)in the hypertension group were larger than those in the control group.The early diastolic blood flow velocity(E)and E/A ratio at the mitral valve were lower in the hypertension group than in the control group(P<0.05).Compared with the control group,the left ventricular global longitudinal peak systolic strain(LVGLS),left ventricular global radial peak systolic strain(LVGRS),left ventricular global circumferential peak systolic strain(LVGCS),and left ventricular global area peak systolic strain(LVGAS)showed a decreasing trend in the hypertension group(P<0.05),and these indicators changed with the increase of hypertension stage(P<0.05).The left ventricular myocardial function parameters(LVEDD,LVESD,E/A,LVGLS,LVGRS,LVGCS,and LVGAS)were related to the stage of essential hypertension(P<0.05).Conclusions Echocardiography is helpful for evaluating left ventricular myocardial function in patients with essential hypertension and can be used to judge the relationship with staging.
