IL-6、hs-CRP、PLT在溃疡性结肠炎患者中的诊断价值

Diagnostic Value of IL-6,Hs-CRP and PLT in Patients with Ulcerative Colitis

目的:探讨白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、血小板计数(PLT)在溃疡性结肠炎(UC)患者中的诊断价值。方法:回顾性分析2021年10月至2023年10月60例活动期UC患者的临床资料,根据UCEIS评分分为轻中度UC组33例与重度UC组27例;根据病理诊断分为有异型增生UC组24例与无异型增生UC组36例。同时选择同期50名健康人作为健康对照组,分析IL-6、hs-CRP、PLT水平变化情况。结果:UC组IL-6、hs-CRP、PLT水平显著高于健康对照组(P<0.05)。重度UC组IL-6、hs-CRP、PLT水平显著高于轻中度UC组(P<0.05)。IL-6、hs-CRP、PLT与UCEIS评分均呈正相关(r分别为0.845、0.826、0.833)。有异型增生UC组IL-6水平显著高于无异型增生UC组(P<0.05),hs-CRP、PLT与无异型增生UC组比较无统计学意义(P>0.05)。IL-6、hs-CRP、PLT预测重度UC的AUC为0.844(95%CI:0.476~0.970)、0.659(95%CI:0.533~0.872)、0.726(95%CI:0.561~0.975)。IL-6预测异型增生的AUC为0.627(95%CI:0.358~0.845),hs-CRP、PLT预测异型增生的AUC分别为0.461、0.394,对异型增生无预测效能。结论:IL-6、hs-CRP、PLT与UC的发生发展有关,可辅助判断病情严重程度,其中IL-6在预测癌前病变方面具有一定的潜能。

Objective To investigate the diagnostic value of interleukin-6(IL-6),hypersensitive C-reac-tive protein(hs-CRP)and platelet count(PLT)in patients with ulcerative colitis(UC).Methods The clinical data of 60 patients with active UC from October 2021 to October 2023 were retrospectively ana-lyzed.According to UCEIS score,they were divided into 33 cases in mild to moderate UC group and 27 ca-ses in severe UC group.According to the pathological diagnosis,there were 24 cases of UC with dysplasia and 36 cases of UC without dysplasia.At the same time,50 healthy people were selected as healthy control group,and the level changes of IL-6,hs-CRP and PLT were analyzed.Results The levels of IL-6,hs-CRP and PLT in the UC group were significantly higher than those in the healthy control group(P<0.05).The levels of IL-6,hs-CRP and PLT in the severe UC group were significantly higher than those in the mild to moderate UC group(P<0.05).IL-6,hs-CRP and PLT were positively correlated with UCEIS scores(r was 0.845,0.826 and 0.833,respectively).The level of IL-6 in the UC group with dysplasia was significantly higher than that in the UC group without dysplasia(P<0.05),and there was no statistical significance in hs-CRP and PLT compared with the UC group without dysplasia(P>0.05).IL-6,hs-CRP and PLT predicted the AUC of severe UC to be 0.844(95%CI:0.476~0.970),0.659(95%CI:0.533~0.872)and 0.726(95%CI:0.561~0.975),respectively.The AUC of IL-6 for predicting dysplasia was 0.627(95%CI:0.358-0.845),and the AUC of hs-CRP and PLT for predicting dysplasia was 0.461 and 0.394,respectively,which had no predictive effect on dysplasia.Conclusion IL-6,hs-CRP and PLT are related to the occurrence and development of UC,and can assist in judging the severity of UC.IL-6 has cer-tain potential in predicting precancerous lesions.
