摘要
目的 探究青少年首发抑郁症血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素13(interleukin 13,IL-13)与躯体症状的相关性。方法 依照纳入及排除标准收集我院在2022年1月-2023年1月进行治疗的120例青少年首发抑郁症病人作为疾病组,另选取同期健康体检人员90例作为健康组。空腹采集健康组和疾病组5mL外周静脉血,室温静置半小时。以4000转每分钟(r/min)的速度离心外周血10min后,取上层清液,酶联免疫吸附测定法检测血清TNF-α、IL-13水平。对比健康组和疾病组的血清TNF-α、IL-13水平,之后根据是否伴随躯体症状,对比不同躯体症状病人与无躯体症状病人血清TNF-α、IL-13水平,并分析TNF-α、IL-13水平与躯体化的相关性。结果 健康组血清TNF-α、IL-13水平明显低于疾病组(P<0.05),99例(82.50%)患者出现躯体化症状。出现失眠、头晕、耳鸣、消化道症状、心悸、出汗、尿频、体重下降症状的躯体化病人血清TNF-α、IL-13水平明显高于未出现躯体症状的病人。血清TNF-α水平与头晕、耳鸣、出汗存在显著正相关,而血清IL-13水平与耳鸣、消化道症状、心悸、体重下降存在显著正相关。结论 青少年首发抑郁症的血清TNF-α、IL-13水平升高,伴躯体症状的抑郁症病人血清TNF-α、IL-13水平会出现进一步升高,血清TNF-α水平与头晕、耳鸣、出汗存在显著正相关,而血清IL-13水平与耳鸣、消化道症状、心悸、体重下降存在显著正相关。
Objective To explore the correlation between serum tumor necrosis factor-α(TNF-α),interleukin-13(IL-13) and somatic symptoms in adolescents with first-episode depression.Methods According to the inclusion and exclusion criteria,120 adolescent first-episode depression patients treated in our hospital from January 2022 to January 2023 were selected as the disease group.In addition,90 health examination personnel in the same period were selected as the health group.Fasting 5m L peripheral venous blood was collected from the health and disease groups and left at room temperature for half an hour.Blood was centrifuged at 4000 RPM for 10min,and the supernatant was removed for determination of serum TNF-α and IL-13 levels by enzyme-linked immunosorbent assay.The serum levels of TNF-α and IL-13 were compared between the two groups,and then the serum levels of TNF-α and IL-13 were compared between patients with different somatic symptoms and patients without somatic symptoms according to whether they were accompanied by somatic symptoms.The correlation between the levels of TNF-α,IL-13 and somatization was analyzed.Results The serum levels of TNF-α and IL-13 in the health group were significantly lower than those in the disease group(P<0.05).Among the participants,99 cases(82.50%) exhibited somatic symptoms.Patients with somatization who experienced insomnia,dizziness,tinnitus,gastrointestinal symptoms,palpitations,sweating,frequent urination,and weight loss showed significantly higher serum levels of TNF-α and IL-13 compared to those without somatic symptoms.Additionally,serum TNF-α levels were significantly positively correlated with dizziness,tinnitus,and sweating,while IL-13 levels were significantly correlated with tinnitus,gastrointestinal symptoms,palpitations,and weight loss.Conclusion In adolescents with first-episode depression,serum levels of TNF-α and IL-13 are elevated,with even higher levels observed in those exhibiting somatic symptoms.TNF-α levels correlate positively with dizziness,tinnitus,and sweating,while IL-13 levels are associated with tinnitus,gastrointestinal symptoms,palpitations,and weight loss.These findings suggest an inflammatory component in depression,especially in patients with somatic manifestations.
