精准注射联合精准牵引治疗腰椎间盘突出症

Precise injection combined with precise traction for the treatment of lumbar disc herniation

目的 探讨精准注射联合精准牵引用于腰椎间盘突出症治疗中的效果.方法 选取我院2022年1月-2024年3月收治的腰椎间盘突出症患者200例,以年龄将患者分为对照组、减压组、注射组和联合组,每组患者数为50例,对照组接受物理因子治疗,减压组接受非手术脊柱减压系统治疗,注射组实施肌骨超声引导下的椎间孔注射治疗,联合组使用肌骨超声引导下的椎间孔注射+非手术脊柱减压治疗,比较四组治疗前、治疗后1周、治疗后2周、治疗后4周和治疗后12周的Oswestry功能障碍指数(ODI)和疼痛情况[视觉模拟评分法(VAS)评分],比较四组患者治疗前和治疗后12周的腰椎MRI检查指标[椎间盘突出指数(DHI)、椎间盘高度(DH)].结果 治疗后1周、治疗后2周、治疗后4周和治疗后12周联合组的ODI和VAS评分显著低于对照组、减压组和注射组(P<0.05).治疗后12周,联合组的DHI值低于其他三组(P<0.05),DH值高于其他三组(P<0.05).结论 精准注射联合精准牵引用于腰椎间盘突出症患者的治疗中可缓解患者的功能障碍,减轻患者的疼痛情况,促进患者腰椎功能的恢复.

Objective To explore the effect of precise injection combined with precise traction in the treatment of lumbar disc herniation.Methods 200 patients with lumbar disc herniation admitted to our department were selected for treatment from January 2022 to March 2024.Patients were divided into a control group,a decompression group,an injection group,and a combination group based on age,with 50 patients in each group.The control group received physical factor treatment,the decompression group received non-surgical spinal decompression system treatment,the injection group received intramuscular ultrasound guided intervertebral foramen injection treatment,and the combination group received intramuscular ultrasound guided intervertebral foramen injection+non-surgical spinal decompression treatment.The Oswestry Dysfunction Index(ODI)and pain levels of the four groups were compared before treatment,1 week after treatment,2 weeks after treatment,4 weeks after treatment,and 12 weeks after treatment.Compare the lumbar MRI examination indicators[intervertebral disc herniation index(DHI)and intervertebral disc height(DH))of four groups of patients before and 12 weeks after treatment using the Visual Analog Scale(VAS).Results The ODI and VAS scores of the combination group after 1 week,2 weeks,4 weeks,and 12 weeks of treatment were significantly lower than those of the control group,decompression group,and injection group(P<0.05).After 12 weeks of treatment,the DHI value of the combination group was lower than the other three groups(P<0.05),and the DH value was higher than the other three groups(P<0.05).Conclusion The combination of precise injection and precise traction can alleviate functional impairment,alleviate pain,and promote the recovery of lumbar spine function in patients with lumbar disc herniation.
