老年2型糖尿病患者TyG指数与糖尿病视网膜病变的相关性分析

The Correlation Analysis of TyG Index and Diabetic Retinopathy in Elderly Type 2 Diabetic Patients

目的观察三酰甘油-葡萄糖指数(TyG指数)在60岁以上的2型糖尿病人群中与糖尿病视网膜病变的相关性。方法收集2019年7月至2022年7月入住安徽医科大学第一附属医院内分泌科的年龄超过60岁的2例糖尿病患者共272例。根据眼底镜结果,分为糖尿病视网膜病变组(DR组,n=64),糖尿病无视网膜病变组(NDR组,n=208)。收集研究对象的一般临床资料,并计算三酰甘油-葡萄糖指数(TyG)。分析两组间各指标的差异及TyG指数与DR的相关性。结果DR组年龄、空腹血糖、TyG指数均较NDR组高,差异有统计学意义。将TyG指数按照四分位数进行分析,结果显示TyG指数水平越高,DR患者的比例越高。根据Logistic回归分析,TyG指数、年龄、糖尿病病程均为DR发生的独立危险因素。结论对糖尿病视网膜病变的高危人群,应重视糖代谢和脂代谢,制定综合管理目标。

Objective To investigate the correlation between triglyceride-glucose index and diabetic retinopathy in type 2 diabetic patients over 60 years old.Methods A total of 272 diabetic patients over 60 years old who were hospitalized in the Endocrinology Department of the First Affiliated Hospital of Anhui Medical University were enrolled between July,2019 and July,2022.According to the ophthalmic examinations results,these patients were divided into the diabetic retinopathy group(DR group,n=64)and non-diabetic retinopathy group(NDR group,n=208).Clinical data of all patients were collected,and triacylglycerol-glucose index(TyG)was calculated.The differences of each index between the two groups and the correlation between TyG index and DR were analyzed.Results Age,FBG and TyG index of the DR group were higher than the NDR group.Analysis of the TyG index by quartiles showed that the higher the level of the TyG index,the higher the prevalence of DR.Logistic regression analysis showed that the TyG index,age,and disease duration were independent risk factors for type 2 diabetic retinopathy.Conclusion For people at high risk of DR,attentions should be paid to glucose metabolism and lipid metabolism,and comprehensive management targets should be determined.
