乙肝肝硬化失代偿期并发肝功能衰竭患者血清Copeptin、HIF-1α、Beclin1检测及其临床意义分析

Detection and Clinical Significance of Serum Copeptin,HIF-1αand Beclin1 in Patients with Decompensated Hepatitis B Cirrhosis and Liver Failure

目的:探讨乙肝肝硬化失代偿期(HBV-DC)并发肝功能衰竭患者血清和肽素(Copeptin)、缺氧诱导因子-1α(HIF-1α)、Beclin1表达水平及其临床意义。方法:HBV-DC并发肝功能衰竭的患者141例纳入病例组,另选同期73例体检的健康对象纳入对照组。根据随访12个月内患者的生存状况将病例组分为存活组和死亡组。检测血清Copeptin、HIF-1α、Beclin1水平。多因素Logistic回归分析HBV-DC并发肝功能衰竭患者死亡的相关影响因素。结果:病例组血清Copeptin、HIF-1α、Beclin1水平高于对照组(P<0.05)。死亡组血清Copeptin、HIF-1α、Beclin1高于存活组(P<0.05)。终末期肝病模型(MELD)评分、Copeptin、HIF-1α、Beclin1升高是HBV-DC并发肝功能衰竭患者死亡的危险因素(P<0.05)。结论:HBV-DC并发肝功能衰竭患者血清Copeptin、HIF-1α、Beclin1水平升高,是患者死亡的独立危险因素。

Objective:To investigate the expression levels and clinical significance of serum copeptin(Copeptin),hypoxia inducible factor-1α(HIF-1α)and Beclin1(Beclin1)in patients with decompensated hepatitis B cirrhosis(HBV-DC)and liver failure.Methods:141 patients with HBV-DC and liver failure were selected as case group,and 73 healthy subjects who underwent physical examination during the same period were included in control group.Case group was divided into survival group and death group according to the survival status of the patients within 12 months of follow-up.The levels of serum Copeptin,HIF-1αand Beclin1 were detected.The related influencing factors of death in patients with HBV-DC and liver failure were analyzed by multivariate Logistic regression.Results:The levels of serum Copeptin,HIF-1αand Beclin1 in case group were higher than those in control group(P<0.05).The serum levels of Copeptin,HIF-1αand Beclin1 in death group were higher than those in survival group(P<0.05).The elevated of model for end-stage liver disease(MELD)score,Copeptin,HIF-1α,and Beclin1 were risk factors for deathin patients with HBV-DC and liver failure(P<0.05).Conclusion:The levels of serum Copeptin,HIF-1αand Beclin1 in patients with HBV-DC and liver failure are increase,which are independent risk factors for death.
