摘要
目的:分析体感音乐疗法结合吞咽动画视听反馈锻炼对帕金森患者吞咽障碍的影响。方法:采用前瞻性研究分析法,将2020年1月至2023年1月郑州市第九人民医院收治的80例帕金森合并吞咽障碍患者以随机数表法分为观察组(n=40)和对照组(n=40),对照组给予常规吞咽功能训练,观察组给予体感音乐疗法结合吞咽动画视听反馈锻炼。对两组患者治疗前3 d(T 0)和治疗完成后3 d(T 1)时的舌骨喉复合体动度、吞咽时间、吞咽功能及T 1时的洼田饮水试验情况进行比较。结果:与对照组相比,T 0时观察组的舌骨/喉向前、上移最大幅度、喉关闭、咽运送、吞咽反应、口腔运送时间、吞咽造影检查(videofluoroscopic swal-lowing study,VFSS)评分、Gugging吞咽功能评估表(gugging swallowing screen,GUSS)评分比较无明显差异(P<0.05),T 1时观察组的舌骨/喉向前、上移最大幅度均增大(P<0.05),T 1时观察组的喉关闭、咽运送、吞咽反应、口腔运送时间均缩短(P<0.05),T 1时观察组的VFSS评分、GUSS评分均升高,观察组洼田饮水试验总优良率92.50%升高(P<0.05)。结论:体感音乐疗法结合吞咽动画视听反馈锻炼可改善舌骨喉复合体动度及吞咽功能。
Objective:To analyze the effect of somatosensory music therapy combined with swallowing animation audiovisual feedback exercise on swallowing disorders in Parkinson's patients.Methods:A prospective study analysis was used to randomly divide 80 Parkinson's patients with swallowing disorders between January 2020 and January 2023 into an observation group(n=40)and a control group(n=40)using a random number table method.The control group received routine swallowing function training,while the observation group received somatosensory music therapy combined with swallowing animation audio-visual feedback training.Compare the movement,swallowing time,swallowing function,and Wada drinking water test results of the hyoid laryngeal complex between two groups of patients at 3 days before treatment(T 0)and 3 days after treatment completion(T 1).Results:Compared with the control group,there was no significant difference(P>0.05)in the observation group's hyoid/larynx forward and upward movement,laryngeal closure,pharyngeal transport,swallowing response,oral transport time,swallowing contrast examination(VFSS)score,and Gugging swallowing function assessment form(GUSS)score at T 0.At T 1,the maximum amplitude of hyoid/larynx forward and upward movement in the observation group increased(P<0.05),while at T 1,the observation group's laryngeal closure,pharyngeal transport,and swallowing response were also significantly increased The oral transport time was shortened(P<0.05),and the VFSS score and GUSS score of the observation group increased at T 1.The total excellent rate of the Wada drinking water test in the observation group increased 92.50%(P<0.05).Conclusion:The combination of somatosensory music therapy and swallowing animation audiovisual feedback exercise can improve the mobility and swallowing function of the hyoid laryngeal complex.
