中间入路与侧方入路腹腔镜手术对结直肠癌患者术后恢复情况及血清肿瘤标志物水平的影响

Impact of Laparoscopic Surgery through Medial and Lateral Approach on Postoperative Recovery and Serum Tumor Marker Levels in Patients with Colorectal Cancer

目的探讨中间入路与侧方入路腹腔镜手术对结直肠癌患者术后恢复情况及血清肿瘤标志物水平的影响。方法将70例结直肠癌患者根据腹腔镜手术入路方式不同分为A组(n=35,行中间入路)和B组(n=35,行侧方入路)。比较两组术后恢复情况、血清肿瘤标志物水平及术后并发症发生率。结果A组肠鸣音恢复时间、肛门排气时间、开始进食时间及住院时间均短于B组(P<0.05);A组术后血清癌胚抗原(CEA)、糖类抗原199(CA199)及糖类抗原125(CA125)水平均低于B组(P<0.05);A组术后并发症发生率为5.71%,与B组的17.14%比较差异无统计学意义(P>0.05)。结论结直肠癌患者行腹腔镜手术时采用中间入路效果优于侧方入路,可明显缩短其术后恢复时间,降低其肿瘤标志物水平,且未明显增加并发症的发生。

Objective To explore the impact of laparoscopic surgery through medial and lateral approach on postoperative recovery and serum tumor marker levels in patients with colorectal cancer.Methods 70 patients with colorectal cancer were divided into group A(n=35,medial approach)and group B(n=35,lateral approach)according to the different approaches of laparoscopic surgery.The postoperative recovery,serum tumor marker levels and incidence of complications were compared between the two groups.Results The recovery time of bowel sounds,anal exhaust time,start eating time and hospitalization time in group A were shorter than those in group B(P<0.05).After surgery,the serum CEA,CA199 and CA125 levels of group A were lower than those of group B(P<0.05).The incidence of postoperative complications in group A was 5.71%,which had no statistical difference with 17.14%in group B(P>0.05).Conclusions For patients with colorectal cancer,the medial approach has better effect than that of lateral approach in laparoscopic surgery,which can significantly shorten the postoperative recovery time,reduce the tumor marker levels,and not significantly increase the incidence of complications.
