溃疡性结肠炎患者血清sTREM-1、ACE2水平与肠道菌群分布的关系

Study on the relationship between serum sTREM-1,ACE2 levels and intestinal microflora distribution in patients with ulcerative colitis

目的检测溃疡性结肠炎患者血清可溶性髓样细胞触发受体1(sTREM-1)、血管紧张素转化酶2(ACE2)表达水平,分析其与肠道菌群分布的关系。方法选取2020年1月-2022年6月辽宁中医药大学附属第三医院收治的128例溃疡性结肠炎患者作为研究对象,根据病情严重程度将其分为轻度组(n=42)、中度组(n=46)和重度组(n=40);根据患者的复发情况分为未复发组(n=97)和复发组(n=31)。比较各组间血清sTREM-1、ACE2水平和肠道菌群含量;Pearson法分析溃疡性结肠炎患者血清sTREM-1、ACE2水平与肠道菌群的相关性。结果溃疡性结肠炎轻、中、重度组血清sTREM-1、肠球菌、肠杆菌含量依次升高(F=35.418、11.503、31.342,P均<0.001),ACE2、乳杆菌、双歧杆菌含量依次降低(F=103.340、32.136、23.100,P均<0.001),3组比较差异均有统计学意义。溃疡性结肠炎患者血清sTREM-1水平与肠球菌、肠杆菌含量成正相关(r=0.483、0.525,P均<0.05),与乳杆菌、双歧杆菌含量成负相关(r=-0.254、-0.376,P均<0.05);血清ACE2水平与肠杆菌含量成负相关(r=-0.397,P均<0.05),与乳杆菌、双歧杆菌含量成正相关(r=0.596、0.482,P均<0.05),与肠球菌无相关性(r=-0.196,P>0.05)。复发组患者血清sTREM-1、肠球菌、肠杆菌含量高于未复发组,ACE2、乳杆菌、双歧杆菌含量低于未复发组,差异均有统计学意义(t=5.914、2.642、6.162、13.384、4.518、4.509,P均<0.05)。结论溃疡性结肠炎患者血清sTREM-1水平显著升高,ACE2水平显著降低,肠道菌群失调,血清sTREM-1、ACE2水平与肠道菌群显著相关。

Objective To investigate the expression levels of soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and angiotensin converting enzyme 2(ACE2)in serum of patients with ulcerative colitis and their relationship with the distribution of intestinal flora.Methods A total of 128 patients with ulcerative colitis admitted to the Third Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2020 to June 2022 were regarded as the study subjects,according to the severity of the disease,they were grouped into mild group(n=42),moderate group(n=46),and severe group(n=40);patients were grouped into non recurrent group(n=97)and recurrent group(n=31)according to their recurrence status.The serum levels of sTREM-1,ACE2 and intestinal microflora were compared among the groups;Pearson method was applied to analyze the correlation between serum levels of sTREM-1,ACE2 and intestinal microflora in patients with ulcerative colitis.Results Serum content of sTREM-1,Enterococcus and Enterobacter increased in mild,moderate and severe groups of ulcerative colitis(F=35.418,11.503,31.342;all P<0.001),ACE2 level,Lactobacillus and Bifidobacterium counts decreased successively(F=103.340,32.136,23.100;all P<0.001).The serum level of sTREM-1 in patients with ulcerative colitis was obviously positively correlated with the contents of Enterococcus and Enterobacter(r=0.483,0.525;all P<0.05),while it was obviously negatively correlated with the contents of Lactobacillus and Bifidobacterium(r=-0.254,-0.376;all P<0.05);the serum ACE2 level was obviously negatively correlated with the content of Enterobacter(r=-0.397,P<0.05),and obviously positively correlated with the contents of Lactobacillus and Bifidobacterium(r=0.596,0.482;all P<0.05).Serum levels of sTREM-1,Enterococcus and Enterobacter in relapsed group were higher than those in non-relapsed group(P<0.05).ACE2 level,Lactobacillus and Bifidobacterium counts were lower than those in the non-recurrence group(t=2.642,6.162,4.518,4.509;all P<0.05).Conclusion The serum level of sTREM-1 in patients with ulcerative colitis was obviously elevated,ACE2 level was obviously decreased,the intestinal flora was out of balance,serum levels of sTREM-1 and ACE2 were obviously correlated with intestinal flora.
