过敏性哮喘患儿嗜酸性粒细胞、血清总IgE水平与肠道菌群的关系

Correlation of eosinophils count and serum total IgE levels with gut microbiota in allergic asthma in children

摘要 目的探讨过敏性哮喘患儿嗜酸性粒细胞(EOS)、血清免疫球蛋白E(Ig E)水平与肠道菌群的关系,为过敏性哮喘患儿的诊治提供参考依据。方法选取河北中医药大学第一附属医院2020年4月-2021年8月收治的98例过敏性哮喘患儿作为研究组,选取同期进行体检的132名健康儿童作为对照组。收集两组受试者一般资料、EOS和IgE水平,并检测其肠道菌群及Alpha多样性(Chao1指数和Shannon指数),采用Spearman分析EOS和Ig E水平与肠道菌群Alpha多样性的相关性。结果两组受试者年龄、性别占比、体质量指数、病程之间差异均无统计学意义(P均>0.05)。研究组EOS计数、EOS百分比和Ig E均高于对照组,差异均有统计学意义(t=11.552、22.767、12.816,P均<0.05)。研究组乳酸杆菌、双歧杆菌、脆弱拟杆菌和罗氏菌水平低于对照组,大肠杆菌水平高于对照组,差异均有统计学意义(t=8.699、16.074、17.876、12.712、7.936,P均<0.05)。研究组Chao1指数和Shannon指数低于对照组,差异均有统计学意义(t=7.376、5.567,P均<0.05)。肠道菌群Alpha多样性Chao1指数和Shannon指数均与血清中EOS计数、EOS百分比和Ig E成负相关性(r_(Chao1指数)=-5.89、-6.35、-6.02,r_(Shannon指数)=-5.73、-6.14、-5.93,P均<0.05)。结论过敏性哮喘患儿EOS和Ig E水平偏高,肠道菌群中乳酸杆菌、双歧杆菌、脆弱拟杆菌和罗氏菌水平偏低,大肠杆菌水平偏高,其Alpha多样性偏低,EOS和Ig E水平与Alpha多样性成负相关。 Objective To explore the correlation of eosinophils(EOS)and immunoglobulin E(IgE)levels with gut microbiota in allergic asthma in children.Methods Ninety-eight children with allergic asthma in the Affiliated Hospital of Hebei University of TCM from April 2020 to August 2021 were selected as study group(n=98),and another 132 healthy children who subjected to physical examination during the same period were enrolled as control group(n=132).General information collection and measurement of EOS and IgE levels were conducted on all subjects.The gut microbiota composition and alpha diversity(Chao1 index and Shannon index)were examined;the correlation of EOS and IgE with gut microbiota alpha diversity was discussed using Spearman rank correlation coefficient.Results The age,sex ratio,body mass index,and duration of disease yielded no statistical difference between two groups(all P>0.05).EOS count,EOS percentage and IgE levels were higher in study group than in control group,with statistical difference(t=11.552,22.767,12.816;all P<0.05).The counts of Lactobacillus,Bifidobacterium,Bacteroides fragilis and Roseburia in study group were lower than those in control group,and the counts of Escherichia coli were significantly higher than those in control group(t=8.699,16.074,17.876,12.712,7.936;all P<0.05).The Chao1 index and Shannon index in study group were both lower than those in control group(t=7.376,5.567;all P<0.05).The Chao1 index and Shannon index of Alpha diversity were negatively correlated with EOS count,EOS percentage and IgE in serum(r_(Chao1 index)=-5.89,-6.35,-6.02;r_(Shannon index)=-5.73,-6.14,-5.93;all P<0.05).Conclusions Children with allergic asthma had elevated EOS and IgE levels,and less gut microbiota Alpha diversity with lower counts of Lactobacillus,Bifidobacterium,Bacteroides fragilis and Roseburia,whereas higher count of Escherichia coli compared with normal controls.In addition,EOS and IgE levels were negatively correlated with Alpha diversity in children with allergic asthma.
