高脂血症合并高血压患者血清载脂蛋白A1及脂蛋白a水平及其与血脂指标的相关性

摘要 目的分析高脂血症合并高血压患者血清载脂蛋白A1(apolipoprotein A1),ApoA1]和脂蛋白a[lipoprotein(a),Lp(a)]水平及其与血脂指标的相关性。方法选取2020年1月至2022年1月新余钢铁集团有限公司中心医院收治的80例高血脂合并高血压患者作为观察组,另选取本院同期50名健康体检者作为对照组。所有研究对象均进行血清Lp(a)、ApoA1及血脂指标[低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)]检测。比较两组血清Lp(a)、ApoA1、LDL-C、HDL-C、TG、TC水平,并采用Pearson相关分析Lp(a)、ApoA1与LDL-C、HDL-C、TG、TC水平的相关性。结果观察组血清HDL-C水平低于对照组,血清TC、TG、LDL-C水平均高于对照组,差异有统计学意义(P<0.05)。观察组血清Lp(a)水平高于对照组,血清ApoA1水平低于对照组,差异有统计学意义(P<0.05);Pearson相关分析结果显示,血清Lp(a)与TC、TG均呈正相关(r>0,P<0.05),与LDL-C、HDL-C无相关性;血清ApoA1与LDL-C、TC均呈正相关(r>0,P<0.05),与HDL-C呈负相关(r<0,P<0.05),但与TG无相关性。结论高血脂合并高血压患者常合并血脂代谢紊乱情况,血清Lp(a)、ApoA1水平有利于疾病诊断,可为判断病情发展提供有效参考依据,有助于改善患者预后。 Objective To investigate the clinical value of serum apolipoprotein A1(ApoA1)and lipoprotein a[Lp(a)]and their correlation with bloodlipid indexes in patients with hyperlipidemia complicated with hypertension.Methods A total of 80 patients with hyperlipidemia combined with hypertension admitted to the Central Hospital of Xinyu Iron&Steel Group Co.,LTD.from January 2020 to January 2022 were selected as the observation group,and another 50 healthy subjects in our hospital during the same times were selected as the control group.Serum Lp(a),ApoA1,blood lipid indexes(low density lipoprotein cholesterol[LDL-C],high density lipoprotein cholesterol[HDL-C],triglyceride[TG]and total cholesterol[TC])were detected in all subjects.The serum levels of Lp(a),ApoA1,LDL-C,HDL-C,TG and TC were compared between the two groups,and the correlation between Lp(a),ApoA1 and LDL-C,HDL-C,TG and TC were analyzed by Pearson correlation analysis.Results The levels of serum HDL-C in the observation group were lower than those in the control group,and the levels of serum TC,TG and LDL-C were higher than those in control group,the differences were statistically significant(P<0.05).The serum Lp(a)level in the observation group was higher than that in the control group,and the serum ApoA1 level was lower than that in the control group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum Lp(a)was positively correlated with TC and TG(r>0,P<0.05),but nocorrelatedwith LDL-C and HDL-C;serum ApoA1 was positively correlated with LDL-C and TC(r>0,P<0.05),negatively correlated with HDL-C(r<0,P<0.05),but no correlated with TG.Conclusion Patients with hyperlipidemia combined with hypertension are often have lipid metabolism disorders,serum Lp(a)and ApoA1 levels are conducive to the diagnosis of the disease,which can provide an effective reference for judging the development of the disease and help to improve the prognosis of patients.
