摘要
目的分析血常规红细胞参数检验在鉴别诊断地中海贫血与缺铁性贫血中的临床应用价值。方法选取2017年10月至2020年3月鞍山市肿瘤医院血液科收治的96例贫血患者作为研究对象,其中45例地中海贫血患者作为A组,51例缺铁性贫血患者作为B组,另选取同期于本院接受体检的60名健康者作为对照组,3组受检者均接受血常规红细胞参数检验,比较分析检测结果。结果3组红细胞计数(red blood cell count,RBC)、血红蛋白(hemoglobin,Hb)、平均红细胞血红蛋白浓度(mean corpuscular hemoglobin concentration,MCHC)、平均红细胞体积(mean corpuscular volume,MCV)、平均红细胞血红蛋白量(mean corpusular hemoglobin,MCH)、红细胞体积分布宽度(redcell volume distribution width,RDW)水平比较差异有统计学意义(P<0.05);A组(α型、β型)与B组的RBC、Hb、MCHC、MCV、MCH水平均低于对照组,RDW水平高于对照组,A组(α型、β型)RBC、Hb水平高于B组,RDW水平低于B组,差异有统计学意义(P<0.05);A组(α型、β型)与B组的MCHC、MCV、MCH水平比较差异无统计学意义;A组α型、β型的RBC、Hb、MCHC、MCV、MCH、RDW水平比较差异无统计学意义。结论血常规红细胞参数检验在鉴别地中海贫血与缺铁性贫血中具有较高的诊断价值,值得临床推广应用。
Objective To analyze the clinical application value of erythrocyte parameter test of blood routine in differential diagnosis of thalasse-mia and iron deficiency anemia.Methods A total of 96 patients with anemia admitted to the hematology department of Anshan Cancer Hospital from October 2017 to March 2020 were selected as research subjects,including 45 patients with thalassemia as the group A,51 patients with iron deficiency anemia as the group B,and 60 healthy people who underwent physical examination in our hospital during the same period as the control group.All subjects in the three groups underwent routine blood erythrocyte parameter test,and the test results were compared and analyzed.Results There were significant differences in red blood cell count(RBC),hemoglobin(Hb),mean corpuscular hemoglobin concentration(MCHC),mean corpuscu-lar volume(MCV),mean corpusular hemoglobin(MCH)and redcell volume distributionwidth(RDW)among the three groups(P<0.05).The RBC,Hb,MCHC,MCV and MCH levels in the group A(αtype,βtype)and the group B were lower than those in the control group,the RDW level were higher than that in the control group,the RBC and Hb levels in the group A(αtype,βtype)were higher than those in the group B,and the RDW level was lower than that in the group B,the differences were statistically significant(P<0.05).There were no significant differences in MCHC,MCV and MCH levels between the group A(αtype,βtype)and the group B.There were no significant differences in RBC,Hb,MCHC,MCV,MCH and RDW levels ofαandβtypes in the group A.Conclusion Erythrocyte parameter test of blood routine has high diagnostic value in differentiating thalasse-mia from iron deficiency anemia and is worthy of clinical application.
