双维度四位一体式干预方案对乳腺癌放疗患者心理状态和疾病应对方式的影响

Effect of two-dimensional four-in-one intervention program on psychological state and disease coping style in breast cancer patients undergoing radiotherapy

目的分析双维度四位一体式干预方案对乳腺癌放疗患者心理状态和疾病应对方式的影响。方法选取2020年1月至2022年1月盐城市第一人民医院放疗科收治的80例乳腺癌放疗患者作为研究对象,采取电脑随机数字表法分为对照组与观察组,每组40例。对照组予以常规护理,观察组在对照组基础上予以双维度四位一体式干预方案。比较护理前后两组心理状态、预期性悲伤、疾病应对方式及创伤后成长水平评分。结果护理后,两组抑郁、焦虑、压力评分均均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05)。护理后,两组愤怒、感知社会支持、悲伤、疾病调整、自我意识、宗教安慰、躯体状况评分及总分均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05)。护理后,两组消极应对方式评分均低于护理前,积极应对方式评分均高于护理前,且观察组消极应对方式评分低于对照组,积极应对方式评分高于对照组,差异有统计学意义(P<0.05)。护理后,两组欣赏生活、与他人关系、精神变化、新的可能性、个人力量评分及总分均高于护理前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论双维度四位一体式干预方案应用于乳腺癌放疗患者中效果显著,不仅能改善患者的心理状态,缓解预期性悲伤程度,还能改善患者疾病应对方式,提高其创伤后成长水平。

Objective To analyze the effect of the two-dimensional four-in-one intervention program on the psychological state and disease cop-ing style in breast cancer patients undergoing radiotherapy.Methods A total of 80 cases of breast cancer radiotherapy patients were admitted to the department of Radiotherapy of Yancheng First People's Hospital from January 2020 to January 2022 were selected as the study subjects,and they were divided into the control group and the observation group according to the computer random number table method,with 40 cases in each group.The control group was given routine nursing,and the observation group was given the two-dimensional four-in-one intervention program.The psychological state,anticipatory grief,disease coping style and post-traumatic growth level before and after nursing were compared between the two groups.Results After nursing,the scores of depression,anxiety and stress of the two groups were lower than before nursing,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).After nursing,the scores of anger,perceived social support,sadness,disease adjustment,self-awareness,religious comfort,physical condition and total score of the two groups were lower than before nursing,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).After nursing,the negative coping style score of the two groups was lower than before nursing,the positive coping style score was higher than before nursing,and the negative coping style score in the observation group was lower than that in the control group,and the positive coping style score was higher than that in the control group,and the differences were statistically significant(P<0.05).After nursing,the scores of appreciation of life,relationship with others,mental changes,new possibilities,personal strength and total scores of the two groups were higher than before nursing,and the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Conclusion The two-dimensional four-in-one intervention program in breast cancer radiotherapy patients has a remarkable effect,which can not only improve the psychological state of the patients,relieve the degree of anticipatory sadness,but also improve the disease coping style and promote the effective improvement of the post-traumatic growth level.
