宫颈薄层液基细胞学联合高危型人乳头瘤病毒检测宫颈上皮内瘤变的价值

The value of thinprep cytologic test combined with highrisk human papillomavirus in the detection of cervical intraepithelial neoplasia

目的探讨宫颈薄层液基细胞学(thinprep cytologic test,TCT)联合高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)检测宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)的价值。方法选取2021年8月至2022年7月景德镇市第二人民医院行CIN筛查的82例女性作为研究对象,均实施TCT、HR-HPV检测,以组织病理学活检为金标准,分析TCT、HR-HPV单项及联合诊断效能。结果阴道镜下组织病理学活检显示,82例筛查女性中炎症性改变28例,CINⅠ患者24例,CINⅡ患者18例,CINⅢ患者9例,浸润癌2例;TCT联合HR�HPV检出阴性27例,阳性55例。TCT联合HR-HPV的诊断准确率高于TCT单一检测,差异有统计学意义(P<0.05);HR-HPV、TCT及TCT联合HR-HPV的诊断灵敏度、特异度比较差异无统计学意义。结论TCT联合HR-HPV筛查CIN具有较高的灵敏度和特异度,可提高检出率,以实现对癌前病变的早期发现及治疗,控制癌变率。

Objective To investigate the value of thinprep cytologic test(TCT)combined with high-risk human papillomavirus(HR-HPV)in the detection of cervical intraepithelial neoplasia.Methods A total of 82 female individuals who underwent CIN screening at The Second People's Hospital of Jingdezhen from August 2021 to July 2022 were selected as the research subjects,all of them were subjected to TCT and HR-HPV tests.With histopathological biopsy as the gold standard,the diagnostic efficacy of single TCT,HR-HPV,and their combination were analyzed.Results Histopathological biopsy under colposcopy showed that there were 28 cases of inflammatory changes,24 cases of CINⅠ,18 cases of CINⅡ,9 cases of CINⅢand 2 cases of invasive cancer in 82 screened women;TCT combined with HR-HPV detected 27 negative cases and 55 positive cases.The diagnostic accuracy of TCT combined with HR-HPV was higher than that of TCT single detection,and the difference was statistically significant(P<0.05);there was no significant difference in the diagnostic sensitivity and specificity of HR-HPV,TCT and TCT combined with HR-HPV.Conclusion Combined detection of TCT and HR-HPV in CIN screening has high sensitivity and specificity,which can improve the detection rate of CIN,realize the early detection and treatment of precancerous lesions,and control the canceration rate.
