摘要
高压电烧伤常导致广泛软组织损伤和严重血管栓塞,破坏组织解剖层次深,特别是合并肌肉、肌腱、神经损伤时,患肢坏死风险高,临床修复难度大,致残率和截肢率高,严重影响患者的生存质量。传统的修复方法多采用反复清创、培养肉芽和二期植皮修复,但受区易发生并发症且外观美学功能差。随着显微外科技术的发展,穿支皮瓣游离移植成为修复电烧伤软组织缺损的重要手段之一。股前外侧穿支皮瓣具有血管恒定、切取范围大、美观性能好且供区损伤小等优点,因此越来越多地应用于电烧伤创面修复。本文对股前外侧穿支皮瓣在电烧伤治疗中应用的研究进展进行综述。
High-voltage electrical burns can often cause extensive soft tissue injury and severe vascular embolism,which can greatly damage the anatomical layers of the tissues.Especially when burns are combined with injuries such as muscle,tendon or nerves,the affected limbs may encounter a high risk of necrosis and difficult clinical repair.The high disability and amputation rates seriously affect the quality of life of patients.Repeated debridement,granulation cultivation and secondary skin grafting are widely used in traditional repairs,while complications are prone to occur,and poor aesthetics are one major drawback of those repairs.As microsurgical techniques develop,free perforator flap transplantation has become one of the important treatments for repairing soft tissue defects in electrical burns.The anterolateral thigh perforator flap has the strengths of constant blood vessels,a large resection range,good aesthetics,and small injury to the donor site.Therefore,it has been increasingly used in the repair of electric burn wounds.This article reviews the research progress on the application of the anterolateral thigh perforator flap in the treatment of electric burns.
