处方流转的驱动与阻碍因素研究——基于医疗机构视角

Study on the Driving and Hindering Factors of Prescription Circulation——From the Persperctive of Medical Institutions

目的:了解处方流转的现状和问题,提出相应的对策。方法:对5个省区市15家医疗机构的相关人员进行半结构化访谈。结果:9家医疗机构处方流转比例小于5%;处方流转的驱动因素有提高患者便利性、节约时间成本、保障患者自主选择权、防止交叉感染、提高药品可及性、降低医院管理成本,处方流转的阻碍因素是利益补偿机制缺乏、医保对接不完善、患者信任度较低、社会药店药事服务能力较弱、数据安全风险和数据互通障碍。结论:医疗机构处方流转效果不佳,应建立合理的利益分配机制,加大政策扶持力度,提升社会药店药事服务能力,加强患者引导和舆论宣传,引入第三方流转平台,结合地域特点设计相关机制与政策,促进处方流转。

Objective To understand the current situation and problems of prescription circulation,and to put forward corresponding countermeasures.Methods The relevant personnel of 15 medical institutions in five provincial-level administrative regions were semi-structured interviewed.Results 9 surveyed medical institutions had a turnover ratio of less than 5%.The driving factors of prescription circulation were improving patient convenience,saving time and cost,ensuring patient independent choice,preventing cross-infection,improving drug accessibility and reducing hospital management cost.The hindering factors of prescription circulation were the lack of interest compensation mechanism,inadequate integration with medical insurance,low patient trust,weak service capacity of social pharmacies,data security risks and data interoperability obstacles.Conclusion Most of the investigated medical institutions have poor prescription transfer effect.It is necessary to establish a reasonable benefit distribution mechanism,increase the policy support for the linkage and coordination of health care,improve the service capacity of social pharmacies,do a good job in patient guidance and public opinion publicity,introduce a third-party circulation platform,and design relevant mechanisms and policies based on regional characteristics to promote prescription circulation.
