等离子射频消融联合下鼻甲骨折外移术治疗鼻中隔偏曲伴慢性肥厚性鼻炎的临床研究

Clinical study of plasma radiofrequency ablation combined with outfracture of inferior turbinate in the treatment of nasal septum deviation with chronic hypertrophic rhinitis

目的 研究等离子射频消融术联合下鼻甲骨折外移术(OIT)在鼻中隔偏曲(NSD)伴慢性肥厚性鼻炎(CHR)患者中的应用效果。方法 选取2021年1月~2023年12月收治的NSD伴CHR患者80例,根据信封法将患者分为研究组(n=40)和对照组(n=40),对照组给予下鼻甲成形术联合OIT治疗,研究组给予等离子射频消融术联合OIT治疗。治疗后连续随访6个月。比较两组患者手术6个月后治疗效果、围术期指标、治疗前和治疗6个月后鼻腔通气指标及术后并发症发生情况。结果 研究组总有效率为95.00%,治疗效果明显优于对照组(总有效率为77.50%)(P <0.05);研究组术中出血量少于对照组,住院时间和鼻外观恢复时间短于对照组(P <0.05);术后6个月,两组鼻腔最小横截面积(MCA)和鼻腔容积(NCV)均升高,鼻腔总阻力(NR)均降低,且研究组MCA、NCV高于对照组,NR低于对照组(P <0.05);术后6个月内研究组并发症发生率为2.50%(1/40),低于对照组的15.00%(6/40)(P <0.05)。结论 相较于下鼻甲成形术联合OIT,等离子射频消融术联合OIT治疗鼻中隔偏曲伴慢性肥厚性鼻炎患者的效果更好,更有利于患者鼻腔通气功能、鼻外观等的恢复,且安全性较高。

Objective To explore the application effect of plasma radiofrequency ablation combined with outfracture of inferior turbinate(OIT)on patients with nasal septum deviation(NSD)complicated with chronic hypertrophic rhinitis(CHR).Methods A total of 80 patients with NSD and CHR in a hospital from January 2021 to December 2023 were selected and divided into the study group(n=40)and the control group(n=40)according to the envelope method.The control group was treated with rhinoplasty of inferior turbinate combined with OIT,while the study group was given plasma radiofrequency ablation combined with OIT.The patients were continuously followed up for 6 months after treatment,the therapeutic effect,perioperative indicators after 6 months of surgery,and the nasal ventilation indicators before treatment and after 6 months of treatment and occurrence of postoperative complications were compared between the two groups.Results The therapeutic effect in the study group was significantly better than that in the control group(total effective rate:95.00%vs.77.50%)(P<0.05).The intraoperative blood loss in the study group was less than that in the control group,and the hospital stay and nasal appearance recovery time were shorter than those in the control group(P<0.05).At 6 months after surgery,the nasal minimal cross-sectional area(MCA)and nasal cavity volume(NCV)in the two groups were increased while the nasal resistance(NR)was decreased,and the MCA and NCV in study group were higher than those in the control group,the NR was lower than that in the control group(P<0.05).The incidence of complications in the study group was 2.50%(1/40)within 6 months after surgery,which was lower than 15.00%(6/40)in the control group(P<0.05).Conclusion Compared with rhinoplasty of inferior turbinate combined with OIT,plasma radiofrequency ablation combined with OIT is more effective in the treatment of NSD with CHR,and the latter one is more conducive to the recovery of nasal ventilation function and nasal appearance,and has higher safety.
