摘要
探讨血管外科微创手术对下肢动脉闭塞症(ASO)的临床治疗效果,以减少开放手术给患者带来的痛苦和降低术后并发症的发生率。回顾性分析近三年在我院接受血管外科微创手术的120例ASO患者的临床资料,根据患者的病变部位、严重程度等采用经皮球囊血管成形术(PTA)、支架植入术(Stenting)等微创技术进行治疗。术前术后均详细记录ASO患者的临床症状、踝肱指数(ABI)、疼痛视觉模拟评分(VAS)及术后并发症发生率,并通过随访评估其治疗效果。结果表明,VAS评分从术前的7.2分降至术后三个月的2.2分,可见患者的疼痛症状得到显著缓解(P<0.01);术后三个月ASO患者的ABI平均指数提高至1.03±0.14分,较术前的0.42±0.15分有显著改善(P<0.01);与术前相比,ASO患者的生活质量虽有一定提高,但与术后的评分比较差异均不显著(P>0.05);手术成功率高达92.5%,经及时处理后的并发症均得到有效控制。通过精准选择适应症、优化手术方案及加强术后管理,进一步提高了手术成功率,减少并发症的发生率,改善患者的预后。血管外科微创手术以其创伤小、恢复快、疗效确切等优势在ASO的治疗中展现出良好的临床应用前景。
The study explores the clinical effect of minimally invasive vascular surgery on lower extremity arterial occlusive disease(ASO),so as to reduce the pain caused by open surgery and the incidence of postoperative complications.The study retrospectively analyzes clinical data of 120 ASO patients undergoing minimally invasive vascular surgery in the hospital in the past three years.The patients are treated with minimally invasive techniques:percutaneous balloon angioplasty(PTA)and Stenting according to the lesion location and severity.The study records clinical symptoms,ankle-brachial index(ABI),visual analogue pain score(VAS)and postoperative complication rate of ASO patients in detail before and after surgery;and evaluates the therapeutic effect through follow-up.The results show that VAS score decreases from 7.2 before surgery to 2.2 three months after surgery,indicating significant relief of pain symptoms(P<0.01).The mean ABI index of ASO patients increases to(1.03±0.14)points 3 months after operation,which is significantly improved compared with(0.42±0.15)points before operation(P<0.01).Compared with preoperative,ASO patients’quality of life is improved to a certain extent,but there is no significant difference compared with postoperative scores(P>0.05).The success rate of operation is as high as 92.5%,and the complications are effectively controlled after timely treatment.Through accurate selection of indications,optimization of surgical plan and strengthening of postoperative management,the success rate of surgery is further improved,the incidence of complications is reduced,and the prognosis of patients is improved.Minimally invasive vascular surgery shows a good clinical application prospect in the treatment of ASO because of its advantages of less trauma,quick recovery and precise curative effect.
