中性粒细胞毒性指数联合碱性磷酸酶染色积分在慢性肾衰竭合并细菌感染患者中的早期诊断价值

摘要 目的分析中性粒细胞毒性指数(TG%)联合中性粒细胞碱性磷酸酶(NAP)染色积分在慢性肾衰竭合并细菌感染患者中的早期诊断价值。方法采取随机抽样方式,选取2022年1月至2023年9月我院肾内科住院的慢性肾衰竭患者(100例,疾病组)及同期的健康体检者(100名,健康组)。慢性肾衰竭患者分为合并细菌感染组(13例)、无细菌感染组(87例)。分析比较3组TG、NAP染色积分的差异;分析比较诊断的灵敏度、特异度、阴性预测值、阳性预测值;分析比较肾衰合并感染组抗生素使用前后两项指标下降幅度。结果合并细菌感染组肾衰竭患者降钙素原、C反应蛋白、TG%、NAP染色积分水平均高于无细菌感染组与健康组;联合检测方式灵敏度、特异度、阳性预测值、阴性预测值均高于单一检测,但差异无统计学意义(P>0.05)。结论TG与NAP染色积分可对慢性肾衰竭患者是否合并细菌感染进行评估,具有较高的诊断价值,还能分析患者受感染的严重程度,为临床治疗提供依据。 Objective To analyze the early diagnostic value of neutrophil cytotoxicity index(TG)combined with alkaline phosphatase(NAP)staining score in patients with chronic renal failure complicated with bacterial infection.Methods A random sampling method was used to select 100 patients with chronic renal failure(disease group)and 100 apparently normal healthy people(healthy group)who were hospitalized in the Department of Nephrology from January 2022 to September 2023.Patients with chronic renal failure were divided into the group with bacterial infection(13 cases)and the group without bacterial infection(87 cases).The differences of TG and NAP staining scores among the three groups were analyzed and compared.The sensitivity,specificity,negative predictive value and positive predictive value of diagnosis were analyzed and compared.The decline of two indicators before and after antibiotic use in renal failure complicated with infection group was analyzed and compared.Results The levels of PCT,C-reactive protein,TG%and NAP staining scores in the group with bacterial infection were higher than those in the group without bacterial infection and the healthy group.The sensitivity,specificity,positive predictive value and negative predictive value of combined detection were higher than those of single detection,but there was no statistical difference(P>0.05).Conclusion TG%and NAP staining score can evaluate whether chronic renal failure patients are complicated with bacterial infection,which has high diagnostic value,and can also analyze the severity of infection in patients,providing basis for clinical treatment.
