摘要
目的:观察体外冲击波联合双氯芬酸二乙胺乳胶剂治疗膝骨关节炎(KOA)患者的效果。方法:选取2022年2月至2024年2月该院收治的120例KOA患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各60例。对照组采用双氯芬酸二乙胺乳胶剂治疗,观察组在对照组基础上联合体外冲击波治疗,比较两组血清炎性因子[肿瘤坏死因子(TNF)-α、白细胞介素(IL)-1β]水平、患侧膝关节最大屈曲度、本体感觉功能指标(平衡轨迹误差值和平衡负重差值)水平,以及西安大略和麦克马斯特大学骨关节炎指数量表(WOMAC)评分。结果:治疗后,两组患侧膝关节最大屈曲度大于治疗前,且观察组大于对照组,两组血清TNF-α、IL-1β等炎性因子水平,平衡轨迹误差值、平衡负重差值等本体感觉功能指标水平,以及疼痛、僵硬、关节日常功能等WOMAC评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:体外冲击波联合双氯芬酸二乙胺乳胶剂治疗KOA患者可增加患侧膝关节最大屈曲度,降低血清炎性因子水平、本体感觉功能指标水平和WOMAC评分,效果优于单纯双氯芬酸二乙胺乳胶剂治疗。
Objective:To observe effects of extracorporeal shock wave combined with Diclofenac diethylamine emulgel in treatment of patients with knee osteoarthritis(KOA).Methods:A prospective study was conducted on 120 patients with KOA admitted to the hospital from February 2022 to February 2024.According to the random number table method,they were divided into observation group and control group,60 cases in each group.The control group was treated with Diclofenac diethylamine emulgel,while the observation group was treated with extracorporeal shock wave on the basis of that of the control group.The levels of serum inflammatory factors[tumor necrosis factor(TNF)-α,interleukin(IL)-1β],the maximumflexion of the affected knee joint,the proprioceptive function indexes(balance trajectory error value and balance weight difference value)levels,and the Western Ontario and McMaster university osteoarthritis index(WOMAC)score were compared between the two groups.Results:After the treatment,the maximumflexion levels of the affected knee joint in the two groups were greater than those before the treatment,and that in the observation group was greater than that in the control group;the levels of serum inflammatory factors such as TNF-αand IL-1β,proprioceptive function indexes such as balance track error value and balance weight difference value,and the WOMAC scores such as pain,stiffness and daily function of joints in the two groups were lower than those before the treatment,and those in the observation group were lower than those in the control group;and the difference was statistically significant(P<0.05).Conclusions:Extracorporeal shock wave combined with Diclofenac diethylamine emulgel in the treatment of the KOA patients can increase the maximumflexion of the affected knee joint,and reduce the levels of serum inflammatory factors,proprioceptive function indexes and WOMAC scores.Moreover,it is superior to simple Diclofenac diethylamine emulgel treatment.
