中药熏洗联合高锰酸钾坐浴治疗湿热下注型混合痔术后患者的效果

Effects of herbal fumigation combined with Potassium permanganate hip bath in treatment of postoperative patients with mixed hemorrhoids of dampness-heat diffusing downward type

目的:观察中药熏洗联合高锰酸钾坐浴治疗湿热下注型混合痔术后患者的效果。方法:选取2020年6月至2022年6月该院收治的60例湿热下注型混合痔术后患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各30例。对照组术后采用高锰酸钾坐浴治疗,观察组在对照组基础上联合中药(自拟方)熏洗治疗,比较两组视觉模拟评分法(VAS)评分、创面水肿程度评分、创面水肿消失时间、创面愈合时间、住院时间和并发症发生率。结果:术后第2、3、7天,两组VAS评分均低于术后第1天,且观察组低于对照组,差异有统计学意义(P<0.05);术后第2、3、7天,两组创面水肿程度评分均低于术后第1天,且观察组低于对照组,差异有统计学意义(P<0.05);观察组创面水肿消失时间、创面愈合时间、住院时间均短于对照组,差异有统计学意义(P<0.05);两组术后14 d内尿潴留、便秘、肛门狭窄等并发症发生率比较,差异均无统计学意义(P>0.05)。结论:中药熏洗联合高锰酸钾坐浴治疗湿热下注型混合痔术后患者可降低VAS评分和创面水肿程度评分,缩短创面水肿消失时间、创面愈合时间和住院时间,效果优于单纯高锰酸钾坐浴治疗。

Objective:To observe effects of herbal fumigation combined with Potassium permanganate hip bath in treatment of postoperative patients with mixed hemorrhoids of dampness-heat diffusing downward type.Methods:A prospective study was conducted on 60 postoperative patients with mixed hemorrhoids of dampness-heat diffusing downward type who were admitted to the hospital from June 2020 to June 2022.According to the random number table method,they were divided into observation group and control group,30 cases in each group.The control group was treated with Potassium permanganate hip bath after the surgery,while the observation group was treated with traditional Chinese medicine(self-made prescription)fumigation on the basis of that of the control group.The pain visual analogue scale(VAS)score,the wound edema score,the wound edema disappearance time,the wound healing time,the hospitalization time,and the incidence of complications were compared between the two groups.Results:On the 2nd,3rd and 7th day after the surgery,the VAS scores of the two groups were lower than those on the 1st day after the surgery,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).On the 2nd,3rd and 7th day after the surgery,the scores of wound edema in the two groups were lower than those on the 1st day after the surgery,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The wound edema disappearance time,the wound healing time and the hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications such as urinary retention,constipation and anal stenosis between the two groups within 14 days after the surgery(P>0.05).Conclusions:Herbal fumigation combined with Potassium permanganate hip bath can reduce the VAS scores and the wound edema scores,and shorten the wound edema disappearance time,the wound healing time and the hospitalization time in the postoperative patients with mixed hemorrhoids of damp-heat betting type.Moreover,it is superior to simple Potassium permanganate hip bath treatment.
