摘要
目的:益生菌联合乳果糖在重症监护病房机械通气肠内营养支持患者中的应用效果分析。方法:选取2019年6月—2022年6月台州市仙居县人民医院重症监护病房收治的机械通气肠内营养支持患者100例作为研究对象,根据随机数字表法分为对照组(n=50)和观察组(n=50)。两组均实施呼吸机相关性肺炎集束化策略及肠内营养支持。对照组给予双歧杆菌三联活菌胶囊治疗,观察组在对照组基础上给予乳果糖口服溶液治疗。比较两组康复进程、呼吸机相关性肺炎发生率、胃肠道不良反应发生情况。结果:观察组机械通气时间、重症监护病房住院时间短于对照组,呼吸机相关性肺炎发生率低于对照组,差异有统计学意义(P<0.001);观察组腹胀发生率低于对照组,差异有统计学意义(P=0.045);两组呕吐、腹泻、消化道出血发生率比较,差异无统计学意义(P>0.05)。结论:益生菌联合乳果糖在ICU机械通气肠内营养支持患者中的应用效果显著,可降低呼吸机相关性肺炎、腹胀发生率,缩短机械通气时间、ICU住院时间。
Objective:To analyze the application effect of probiotics combined with lactulose in mechanically ventilated patients with enteral nutritional support in intensive care unit.Methods:A total of 100 mechanically ventilated patients with enteral nutritional support admitted to intensive care unit of Xianju People's Hospital in Taizhou from June 2019 to June 2022 were selected as the study subjects.They were divided into control group(n=50)and observation group(n=50)according to the random number table method.Ventilator-associated pneumonia bundle strategy and enteral nutrition support were implemented in both groups.The control group was treated with Live Combined Bifidobacterium,Lactobacillus and Enterococcus Capsules,and the observation group was treated with Lactulose Oral Solution on the basis of control group.The rehabilitation process,incidence of ventilator-associated pneumonia and incidence of gastrointestinal adverse reactions were compared between the two groups.Results:The duration of mechanical ventilation and length of intensive care unit stay in the observation group were shorter than those in the control group,and the incidence of ventilator-associated pneumonia in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.001).The incidence of abdominal distension in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.045).There was no significant difference in the incidences of vomiting,diarrhea and gastrointestinal bleeding between the two groups(P>0.05).Conclusion:The application of probiotics combined with lactulose in mechanically ventilated patients with enteral nutritional support in intensive care unit has a significant effect,which can reduce the incidences of ventilator-associated pneumonia and abdominal distension,and shorten the duration of mechanical ventilation and length of ICU stay.
