基于横断面研究和改良德尔菲法制定《早中期结直肠癌常规治疗后中医干预指南》的临床问题

Clinical Issues in Developing Clinical Guidelines for TCM Intervention after Routine Treatment of Western Medicine for Early to Mid-stage Colorectal Cancer Based on Cross-sectional Study and Modified Delphi Method

目的:构建及明确纳入《早中期结直肠癌常规治疗后中医干预指南》的临床问题,为制定指南及优化中国早中期结直肠癌治疗模式提供数据基础。方法:先采用横断面研究的方法构建指南临床问题清单,然后应用改良德尔菲法确定临床问题,并计算积极系数、变异系数、协调系数等,进行质量控制。结果:共收集到113份有效问卷,构建了含有9个临床问题的初稿。再通过13名专家经过两轮德尔菲法讨论修订后,形成6个临床问题纳入指南,专家对临床问题的积极性及一致性高。结论:应用改良德尔菲法制定的指南临床问题代表性强、意见趋同性好,能为后续制定指南奠定基础。

Objective:To provide the data for the following establishment of the guidelines for TCM intervention after routine treatment of Western medicine for early to mid-stage colorectal cancer and the optimization of therapeutic mode for early to mid-stage colorectal cancer in China by constructing and explicitly incorporating clinical issues in the guideline.Methods:Cross-sectional studies were adopted to construct the list of clinical problems,and then modified Delphi method was applied to confirm clinical problems,the positive coefficients,variation coefficients and coordination coefficients were calculated for quality control.Results:A total of 113 valid questionnaires were collected to construct the first draft with nine clinical problems,the draft was then revised by 13 experts after two rounds of Delphi method discussions,consequently resulting in six clinical questions for inclusion in the guideline,with a high degree of positivity and consistency among the experts on the clinical questions.Conclusion:The guideline-related clinical issues developed using the modified Delphi method have strong representativeness and good convergence of opinions,which lays the foundation for the establishment of the guideline in the future.
