基于解剖列车理论应用冲击波循经穴位治疗跖筋膜炎45例

Treating 45 Cases of Plantar Fascitis with Shock Wave via Acupoints Based on the Theory of Anatomy Trains

摘要 目的:观察基于解剖列车理论应用体外发散式冲击波循经穴位治疗跖筋膜炎(plantar fascitis,PF)的临床疗效。方法:将90名PF患者随机分为观察组和对照组,每组45例。对照组予以冲击波痛点常规治疗,观察组予以体外冲击波循足少阴肾经、足太阳膀胱经穴位治疗。结果:治疗前两组疼痛评分及踝与后足功能评分差异无统计学意义(P>0.05);治疗4周时、治疗后半年时,两组疼痛评分均明显下降(P<0.05),且观察组下降更明显(P<0.05);治疗4周时、治疗后半年时,两组踝与后足功能评分均明显上升(P<0.05),且观察组上升更明显(P<0.05)。总有效率观察组[95.6%(43/45)]优于对照组[82.2%(37/45)](P<0.05)。结论:常规体外发散式冲击波痛点及循经穴位治疗均能有效缓解PF患者行走疼痛症状,提高踝与后足功能评分,且基于解剖列车理论使用冲击波循经穴位治疗效果更明显、疗效更持久。 Objective:To observe curative effects of extracorporeal disseminated shock wave via acupoints in the treatment of plantar fascitis(PF)based on the theory of anatomy trains.Methods:A total of 90 PF patients were randomized into the observation group and the control group,45 cases in each group.The control group was treated with conventional therapy of shock wave via pain point,and the observation group accepted extracorporeal shock wave via the acupoints of the kidney meridian of foot-Shaoyin,and bladder meridian of foot-Taiyang.Results:The difference had no statistical menaing in VAS scores,ankle and hindfoot function scores before the treatment between the two groups(P>0.05);VAS scores were obviously lower in the two groups after four weeks of the treatment and at six months after the treatment(P<0.05),and the decrease of the observation group was more evident(P<0.05);ankle and hindfoot function scores were elevated in the two weeks after four weeks of the treatment and at six months after the treatment(P<0.05),and the increase of the observation group was more apparent(P<0.05).Total effective rate of the observation group was[95.6%(43/45],better than[82.2%(37/45]of the control group(P<0.05).Conclusion:Conventional therapy of extracorporeal disseminated shock wave via pain point,and extracorporeal disseminated shock wave via acupoints could effectively relieve the symptoms of walking pain,raise ankle and hindfoot function scores,and more significant and long-lasting clinical effects could be achieved by using shock wave via acupoints based on the theory of anatomy trains.
