摘要
目的为探讨老年人胰岛素抵抗及腹型肥胖的发生与初步筛查为高血压的关联,特开展本次研究。方法对重庆市渝北区辖区内老年人开展基本信息采集、血生化及体格检查,共纳入研究对象136403人,采用IBM SPSS 26.0进行χ~2检验完成组间比较,使用分位数回归和二元logistic回归分析探讨代表胰岛素抵抗及腹型肥胖的TyG及相关指数与血压值的关系以及高血压患病的多因素分析。结果TyG及相关指数与高血压密切相关,TyG第4、3、2四分位组诊断为高血压的概率分别是第1四分位组的1.503、1.277、1.120倍(P<0.001),随着分位数分组越大,TyG及相关指数对高血压的影响越大。TyG及相关指数正向影响收缩压、舒张压的变化,对收缩压的作用程度高于舒张压。结论在重庆市渝北区老年人群中,TyG指数及腹型肥胖与高血压患病存在密切联系,TyG指数越高、腹型肥胖越严重,诊断为高血压的可能性越高。本研究为多种慢性病共同干预的重要性提供了数据支持。
Objective This study was conducted to investigate the relationship between insulin resistance, abdominal obesity and hypertension.Methods Basic information collection, blood biochemistry and physical examination were carried out for 136 403 people over 65 years old in Yubei District of Chongqing.IBM SPSS 26.0 was used to perform χ^(2) test to complete the comparison between groups.Quantile regression and binary logistic regression analysis were used to investigate the relationship between TyG and related indexes representing insulin resistance and abdominal obesity and blood pressure, as well as the the relationship between TyG and initial diagnosis of hypertension.Results TyG and related indexes were positively correlated with systolic and diastolic blood pressure, the probability of being diagnosed with hypertension in the 4th, 3rd and 2nd quartile groups of TyG was 1.503,1.277 and 1.120 times that of the 1st quartile group, respectively(P<0.001).The larger the quantile group, the greater the influence of TyG and related indexes on hypertension.TyG and related indexes positively affect the changes of systolic and diastolic blood pressure, and the effect of TyG on systolic blood pressure is higher than that of diastolic blood pressure.Conclusion Among the elderly over 65 years old in Yubei District of Chongqing, insulin resistance and abdominal obesity were closely related to hypertension.The more severe insulin resistance and abdominal obesity were, the higher the likelihood of developing hypertension.This study provided data support for the importance of joint intervention for multiple chronic diseases in the population.
