首次非诱发性癫痫发作的复发风险及危险因素研究

Study on the Recurrence Risk and Risk Factors of First Unprovoked Seizures

目的:研究首次非诱发性癫痫发作患者的复发风险,并探究首次非诱发性癫痫患者复发的危险因素。方法:回顾性分析2021年1月至2023年6月于九江市第一人民医院经4 h视频脑电图(VEEG)和脑三维磁共振成像(MRI)诊断为首次非诱发性癫痫发作的106例患者,统计患者的总体累计复发率,后通过logistic回归分析影响复发的危险因素。结果:106例患者中,40例患者出现癫痫复发情况,总体累积复发风险为37.74%;多因素logistic回归结果显示,VEEG异常为首次非诱发性癫痫发作复发的危险因素(P<0.05);VEEG癫痫波异常是首次非诱发性癫痫发作复发的危险因素(P<0.05)。结论:本研究所选患者中,首次非诱发性癫痫发作患者的总体累积复发风险为37.74%,VEEG癫痫波异常为首次非诱发性癫痫发作患者复发的危险因素。

Objective Research on the recurrence risk in patients with first unprovoked seizures and exploration of risk factors for Rrecurrence in first unprovoked seizures patients.Methods Retrospective analysis of 106 patients diagnosed with first unprovoked seizure by 4-hour video electroencephalogram(VEEG)and three-dimensional brain magnetic resonance imaging(MRI)at Jiujiang NO.1 People's Hospital from January 2021 to June 2023.The overall cumulative recurrence rate of the patients was statistically analyzed,followed by logistic regression analysis to identify the risk factors affecting recurrence.Results Among the 106 patients,40 experienced recurrence of epilepsy,with an overall cumulative recurrence risk of 37.74%.The results of multivariate logistic regression analysis indicated that VEEG abnormality was a risk factor for recurrence of first unprovoked seizures(P<0.05);and the presence of abnormal epileptic waves on VEEG was a risk factor for recurrence of first unprovoked seizures(P<0.05).Conclusion In the patients selected for this study,the overall cumulative recurrence risk for patients with first unprovoked seizures was 37.74%,and the abnormality of VEEG epileptic waves was a risk factor for recurrence in patients with first unprovoked seizures.
