摘要
目的 探讨血清壳多糖酶3样蛋白1(CHI3L1)水平与2型糖尿病(T2DM)患者合并代谢相关脂肪性肝病(MAFLD)的关系。方法 按照1∶1比例选取2020年1月至2023年12月河北省唐山南湖医院收治的T2DM合合并MAFLD患者(MAFLD组)和未合并MAFLD的T2DM患者(非MAFLD组)各120例,采用酶联免疫吸附法检测2组患者血清CHI3L1水平,并收集2组患者基线资料。采用多因素条件Logistic回归模型分析CHI3L1与T2DM患者合并MAFLD的关系,绘制受试者工作特征(ROC)曲线,分析CHI3L1对T2DM合并MAFLD的预测价值。结果 与非MAFLD组比较,MAFLD组血清CHI3L1水平升高(P <0.05)。调整混杂因素后,CHI3L1升高为T2DM患者合并MAFLD的独立危险因素(P <0.05)。血清CHI3L1水平预测T2DM患者合并MAFLD的曲线下面积(AUC)为0.803,敏感度为70.83%,特异度为88.33%。结论 T2DM患者血清CHI3L1水平升高是合并MAFLD的独立危险因素,并对T2DM患者合并MAFLD具有一定的预测价值。
Objective To investigate the relationship between serum chitinase 3-like 1(CHI3L1)levels and metabolism-associated fatty liver disease(MAFLD)in patients with type 2 diabetes mellitus(T2DM).Methods 120 T2DM patients with MAFLD(MAFLD group)and 120 T2DM patients without MAFLD(non-MAFLD group)were selected according to a 1:1 ratio.Serum CHI3L1 levels were detected by enzyme-linked immunosorbent assay.A multiple-factor conditional logistic regression model was used to analyze the relationship between CHI3L1 and MAFLD in T2DM patients and to draw receiver operating characteristic(ROC)curves.Results Serum CHI3L1 levels were elevated in the MAFLD group compared with the non-MAFLD group(P<0.05).After adjusting for confounders,elevated CHI3L1 was an independent risk factor for concurrent MAFLD in T2DM patients(P<0.05).The area under the curve(AUC)of serum CHI3L1 level for predicting concurrent MAFLD in T2DM patients was 0.803(95%CI:0.747-0.851),with a sensitivity of 70.83%and a specificity of 88.33%.Conclusion Elevated serum CHI3L1 levels in T2DM patients are an independent risk factor for concurrent MAFLD and have predictive value for T2DM patients with MAFLD complications.
