远端桡动脉路径DSA指导下弹簧圈栓塞介入治疗颅内动脉瘤疗效及安全性评价

The Evaluation of Efficacy and Safety of Coil Embolization in Treatment of Intracranial Aneurysms via Distal Trans-Radial Artery under Guidance of DSA

目的 探讨远端桡动脉路径(dTRA)全脑血管造影术(DSA)指导下弹簧圈栓塞介入治疗颅内动脉瘤的疗效。方法 回顾性纳入2021年12月至2023年12月于皖南医学院附属太和医院行远端桡动脉路径DSA指导下弹簧圈栓塞介入治疗的90例颅内动脉瘤患者作为观察组,另选同期经股动脉路径治疗的90例患者作为对照组。观察2组患者手术相关指标,术后3、6、12个月改良Rankin量表评分及术后并发症发生情况。结果 观察组穿刺时间及手术时间长于对照组,但住院时间短于对照组(均P <0.05),2组首次穿刺成功例数比较差异无统计学意义(P> 0.05)。2组术后3、6、12个月改良Rankin量表评分同期比较差异无统计学意义(P> 0.05)。观察组术后并发症总发生率低于对照组(P <0.05)。结论 远端桡动脉路径DSA指导下弹簧圈栓塞介入治疗颅内动脉瘤的疗效与传统股动脉路径疗效相当,且能够缩短患者住院时间,降低术后并发症发生率,安全性较高。

Objective To investigate the efficacy of coil embolization in the treatment of intracranial aneurysms under the guidance of distal radial artery routing(dTRA)angiography(DSA).Methods A total of 90 patients with intracranial aneurysms who received coil embolization interventional therapy via distal radial artery under DSA guidance in our hospital from December 2021 to December 2023 were retrospectively included as the observation group,and another 90 patients who received femoral artery route therapy during the same period were selected as the control group.The operative indexes,operative efficacy,and postoperative complications were observed.Results The puncture time and operation time in the observation group were longer than those in the control group,but the hospitalization time was shorter than those in the control group(all P<0.05).There was no statistical significance in the number of successful first-time puncture cases between the two groups(P>0.05).There was no significant difference in the score of the modified Rankin scale between the two groups at 3 months,6 months,and 12 months after surgery(P>0.05).The total complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion The therapeutic effect of coil embolization in the treatment of intracranial aneurysms via distal radial artery under DSA guidance is comparable to that of the traditional femoral artery approach,and this approach can shorten the length of hospital stay and reduce the incidence of postoperative complications,with high safety.
