缩宫素辅助高强度聚焦超声在治疗单发及多发性子宫肌瘤中的应用

摘要 目的比较研究缩宫素辅助下高强度聚焦超声(HIFU)治疗在单发及多发性子宫肌瘤中的特征参数及临床疗效。方法回顾性分析接受缩宫素辅助下HIFU治疗的子宫肌瘤患者64例,其中单发性子宫肌瘤组37例,多发性子宫肌瘤组27例,比较两组患者基本情况;缩宫素辅助下HIFU消融后,比较两组患者消融参数、肌瘤消融率、满意消融率以及不良反应。结果缩宫素辅助HIFU治疗单发及多发性子宫肌瘤的消融率分别为95.51%和93.58%(P<0.05)。多发性子宫肌瘤组辐照时间、治疗时间和治疗能量均高于单发性子宫肌瘤组(P<0.05)。能效因子(EEF)和满意消融率差异无统计学意义(P>0.05),且无明显不良反应。结论缩宫素辅助HIFU治疗单发性和多发性子宫肌瘤均能达到满意的消融效果,虽然治疗多发性子宫肌瘤需更多能量和治疗时间,但未产生明显不良反应,证实缩宫素辅助HIFU是一种安全有效的治疗多发性肌瘤方法。 Objective The study was to evaluate the characteristic parameters and clinical efficacy of high intensity focused ultrasound(HIFU)assisted by oxytocin in the treatment of single and multiple uterine fibroid.Methods Sixty-four patients with uterine fibroid who underwent oxytocin assisted HIFU ablation were retrospective researched,which were divided into single uterine fibroid group(37 cases)and multiple uterine fibroid group(27 cases),and the basic data of the two groups were compared.The ablation parameters,fibroid ablation rate,satisfactory ablation rate and adverse reactions were compared between the two groups after HIFU ablation assisted by oxytocin.Results The ablation rates of single and multiple uterine fibroids treated with HIFU were 95.51%and 93.58%,respectively(P<0.05).The irradiation time,treatment time and treatment energy of multiple uterine fibroids group were higher than those of single uterine fibroids group(P<0.05).There was no significant difference in energy efficiency factor(EEF)and satisfactory ablation rate(P>0.05),and no obvious adverse reactions.Conclusions Oxytocin assisted HIFU can achieve satisfactory ablation effect in the treatment of single and multiple uterine fibroid.Although the treatment of multiple uterine fibroid requires more energy and treatment time,there are no obvious adverse reactions.It is confirmed that oxytocin assisted HIFU is a safe and effective method for the treatment of multiple uterine fibroid.
