助产士全程导乐陪伴护理在无痛分娩产妇中的应用效果研究

Study on the Effect of Midwife’s Whole-Course Doula Nursing in Painless Delivery

目的分析产妇无痛分娩中助产士全程导乐陪伴护理的应用价值。方法以三明市第二医院136例无痛分娩产妇为研究对象,筛选于2020年1月至2023年12月间,按护理模式分组,行常规护理为对照组,应用助产士全程导乐陪伴护理为观察组,每组均68例患者,对比护理效果。结果除第三产程时间基本相同外,观察组均比对照组有更短的第一、第二产程及总产程时间(P<0.05);观察组第一、二、三产程视觉模拟评分(VAS)低于对照组(P<0.05);观察组阴道分娩率较对照组高,而剖宫产率低于对照组(P<0.05);观察组产后出血、新生儿窒息、胎儿宫内窘迫的发生率均低于对照组(P<0.05);护理后,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于护理前且观察组更低(P<0.05)。结论产妇无痛分娩中应用助产士全程导乐陪伴护理后,可加速产程进展,减轻疼痛程度,促进阴道分娩顺利完成,降低剖宫产率,改善妊娠结局。

Objective To analyze the application value of midwife’s doula accompanying nursing during parturient painless delivery.Methods A total of 136 women with painless delivery in Sanming Second Hospital were selected as the research objects from January 2020 to December 2023.They were divided according to nursing mode,with routine nursing as the control group and doula accompanied nursing by midwives as the observation group,with 68 patients in each group,and the nursing effect was compared.Results Except the third stage of labor,the observation group had shorter first stage,second stage and total labor time than the control group(P<0.05).Visual analogue scores(VAS)of the first,second and third stages of labor in the observation group were lower than those in the control group(P<0.05).The vaginal delivery rate of the observation group was higher than that of the control group,but the cesarean section rate was lower than that of the control group(P<0.05).The incidence rates of postpartum hemorrhage,neonatal asphyxia and fetal distress in observation group were lower than those in control group(P<0.05).After nursing,SAS and SDS scores in both groups were lower than before nursing,and lower in observation group(P<0.05).Conclusions The application of doula accompanied nursing in painless delivery can accelerate the progress of labor,reduce the degree of pain,promote the successful completion of vaginal delivery,reduce the rate of cesarean section,improve the outcome of pregnancy.
