分娩球联合自由体位护理的应用对初产妇分娩母婴结局和镇痛效果的影响

To Explore the Effect of Delivery Ball Combined with Free Position Nursing on Maternal and Infant Outcomes and Labor Analgesia in Primiparas

目的探究初产妇分娩中行分娩球联合自由体位助产对改善分娩镇痛及母婴结局的作用。方法选取泉州医学高等专科学校附属人民医院2023年7月至2024年4月收治的足月初产妇120例纳入研究,经随机数字表法分为两组,对照组(60例)行常规助产模式,研究组(60例)在对照组基础上行分娩球联合自由体位助产模式,比较两组总产程时间、分娩控制感、产时疼痛及母婴结局。结果研究组初产妇的总产程时间短于对照组,分娩控制量表(LAS)评分高于对照组(均P<0.05)。研究组产时疼痛程度评分中视觉模拟量表(VAS)、语言反应评分(VRS)与现实疼痛强度评分(PPI)均低于对照组(均P<0.05)。两组母婴结局比较,研究组新生儿生后1 min的Apgar评分更高,会阴侧切率与产后出血率更低(均P<0.05)。结论通过采取分娩球联合自由体位助产方式,初产妇的总产程时间缩短,有利于缓解产妇产时疼痛、提升分娩控制感,改善母婴结局。

Objective To explore the effect of delivery ball combined with free position midwifery on labor analgesia and maternal and infant outcomes in primipara.Methods A total of 120 full-term primiparas admitted to the People’s Hospital Affiliated to Quanzhou Medical College from July 2023 to April 2024 were selected and randomly divided into two groups.The control group(n=60)received conventional midwifery mode,and the study group(n=60)received delivery ball combined with free position midwifery mode on the basis of the control group.The total duration of labor,sense of delivery control,intrapartum pain and maternal and infant outcomes were compared between the two groups.Results The total duration of labour for primiparous women in the study group was shorter than that of the control group,and the labour agentry scale(LAS)score was higher than that of the control group(all P<0.05).The visual analogue scale(VAS),verbal response scale(VRS)and present pain intensity(PPI)of the pain level score during labour were lower in the study group than in the control group(all P<0.05).Comparing the maternal and infant outcomes between the two groups,the study group had a higher Apgar score at 1 min after birth,and a lower rate of perineal circumcision and postpartum haemorrhage(all P<0.05).Conclusions By taking delivery ball combined with free position midwifery,the total labor time of primipara is shortened,which is conducive to relieving maternal pain during labor,improving the sense of delivery control,and improving maternal and infant outcomes.
