重组人脑利钠肽联合左西孟旦治疗重症心力衰竭的效果分析

Analysis of the Effect of Recombinant Human Brain Natriuretic Peptide Combined with Levosimendan in the Treatment of Critical Heart Failure

目的:分析重组人脑利钠肽(rh-BNP)联合左西孟旦治疗重症心力衰竭的效果。方法:选取2022年1月—2024年4月南充市西充县人民医院收治的重症心力衰竭患者60例为研究对象,随机纳入左西孟旦组(n=30)与联合组(n=30)。左西孟旦组采用左西孟旦治疗,联合组采用rh-BNP联合左西孟旦治疗。比较两组心功能、血清炎性因子水平、心肌酶水平、不良反应发生情况。结果:治疗后,两组N末端B型利钠肽前体水平降低,左室射血分数升高,联合组优于左西孟旦组(P<0.05);治疗后,两组肿瘤坏死因子-α、超敏C反应蛋白、白细胞介素-6水平降低,联合组低于左西孟旦组(P<0.05);治疗后,两组乳酸脱氢酶、肌酸激酶同工酶、肌酸激酶水平降低,联合组低于左西孟旦组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:rh-BNP联合左西孟旦可以有效改善重症心力衰竭患者的心功能和心肌酶水平,减轻炎性反应,并具有良好的安全性。

Objective:To analyze the effect of recombinant human brain natriuretic peptide(rh-BNP)combined with levosimendan in the treatment of critical heart failure.Methods:Sixty patients with severe heart failure admitted to Nanchong Xichong County People's Hospital from January 2022 to April 2024 were selected for the study,and were randomly included in the levosimendan group(n=30)and the combined group(n=30).The levosimendan group was treated with levosimendan,and the combined group was treated with rh-BNP combined levosimendan.Cardiac function,serum inflammatory factor levels,cardiac enzyme levels,and the occurrence of adverse reactions were compared between the two groups.Results:After treatment,the level of N-terminal Btype natriuretic peptide precursor decreased and left ventricular ejection fraction increased in both groups,and combined group was superior to levosimendan group(P<0.05);after treatment,the levels of tumor necrosis factor-α,ultrasensitive C-reactive protein and interleukin-6 decreased in both groups,and combined group was lower than levosimendan group(P<0.05);after treatment,the levels of lactate dehydrogenase,creatine kinase isoenzyme and creatine kinase were reduced in both groups,and combined group was lower than that of levosimendan group(P<0.05);the difference in the total incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion:rh-BNP combined with levosimendan can effectively improve the cardiac function and cardiac enzyme levels of patients with severe heart failure,reduce the inflammatory response,and have good safety.
