内镜下低温等离子扁桃体部分切除术伴腺样体切除术治疗儿童阻塞性睡眠呼吸暂停低通气综合征的临床效果观察

Clinical Effect of Endoscopic Low-temperature Plasma Partial Tonsillectomy with Adenoidectomy in the Treatment of Obstructive Sleep Apnea Hypopnea Syndrome in children

目的:观察内镜下低温等离子扁桃体部分切除术伴腺样体切除术治疗儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床效果。方法:选取2023年4月—2024年3月六盘水市第二人民医院收治的OSAHS患儿100例纳入研究,随机分为对照组(n=50,行传统扁桃体完全剥离术伴腺样体切除术)、观察组(n=50,行内镜下低温等离子扁桃体部分切除术伴腺样体切除术)。对比两组手术指标、术后并发症发生情况、睡眠质量。结果:观察组手术时间短于对照组,术中出血量少于对照组,术后视觉模拟评分法评分低于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.027);治疗后,两组呼吸紊乱指数下降,最低血氧饱和度、平均血氧饱和度升高,观察组优于对照组(P<0.05)。结论:内镜下低温等离子扁桃体部分切除术伴腺样体切除术治疗儿童OSAHS的效果良好,能缩短手术时间,减少术中出血量,减轻术后疼痛,降低术后并发症发生率,提高睡眠质量。

Objective:To observe the clinical effect of endoscopic low-temperature plasma partial tonsillectomy with adenoidectomy on obstructive sleep apnea hypopnea syndrome(OSAHS)in children.Methods:A total of one hundred OSAHS children admitted to Liupanshui Second People's Hospital from April 2023 to March 2024 were included in the study,and were randomly divided into control group(n=50,traditional complete tonsillectomy with adenoidectomy)and observation group(n=50,endoscopic low-temperature plasma partial tonsillectomy with adenoidectomy).The surgical indexes,postoperative complications and sleep quality were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the score of the postoperative visual simulation score was lower than that of the control group(P<0.05);the complication rate of observation group was lower than that of control group(P=0.027);after treatment,the respiratory disorder index decreased,the minimum and average blood oxygen saturation increased,and the observation group was better than the control group(P<0.05).Conclusion:Endoscopic low-temperature plasma partial tonsillectomy with adenoidectomy is effective in the treatment of OSAHS in children,which can shorten operation time,reduce intraoperative blood loss,alleviate postoperative pain,reduce the incidence of postoperative complications,and improve sleep quality.
