蛇毒血凝酶联合奥美拉唑和垂体后叶素治疗急性上消化道出血的效果观察

Effect of Snake Venom Hemagglutinin Combined with Omeprazole and Pituitrin on Acute Upper Gastrointestinal Bleeding

目的:观察蛇毒血凝酶、奥美拉唑、垂体后叶素联合治疗急性上消化道出血的临床效果。方法:选取2021年1月—2024年1月榆林市靖边县人民医院收治的急性上消化道出血患者86例为研究对象,遵循随机数字表法分组原则,将患者随机纳入对照组(n=43,给予奥美拉唑联合垂体后叶素治疗)、观察组(n=43,给予奥美拉唑、垂体后叶素联合蛇毒血凝酶)治疗。观察两组疗效、出血停止时间、腹痛缓解时间、总住院时间、凝血功能、不良反应发生情况。结果:观察组治疗总有效率高于对照组(P=0.040);观察组出血停止时间、腹痛缓解时间、总住院时间短于对照组(P<0.05);治疗后,两组活化部分凝血活酶时间、凝血酶时间缩短,D-二聚体水平降低,观察组优于对照组(P<0.05);两组不良反应发生率比较,无统计学差异(P>0.05)。结论:蛇毒血凝酶、奥美拉唑、垂体后叶素联合治疗急性上消化道出血的临床效果确切,能够迅速止血、缓解腹痛,改善凝血功能,缩短住院时间,且未增加不良反应发生风险。

Objective:To observe the clinical effect of snake venom hemagglutinin,omeprazole and pituitrin in the treatment of acute upper gastrointestinal bleeding.Methods:A total of eighty-six patients with acute upper gastrointestinal hemorrhage admitted to Yulin Jingbian County People's Hospital from January 2021 to January 2024 were selected as the study objects.Patients were randomly included in control group(n=43,omeprazole combined with pituitrin)and observation group(n=43,omeprazole combined with pituitrin combined with snake venom hemagglutinin)according to the random-number table method.The efficacy,bleeding stop time,abdominal pain relief time,total hospital stay,coagulation function and adverse reactions were observed in the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.040);bleeding stop time,abdominal pain relief time and total hospital stay in observation group were shorter than those in control group(P<0.05);after treatment,the activated partial thromboplastin time and thrombin time were shortened and the D-dimer level was decreased in both groups,and the observation group was better than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of snake venom hemagglutinin,omeprazole and pituitrin in the treatment of acute upper gastrointestinal bleeding has definite clinical effects,which can quickly stop bleeding,relieve abdominal pain,improve coagulation function,shorten hospital stay,and do not increase the risk of adverse reactions.
