腹腔镜下中低位直肠癌根治术中应用吻合口加固缝合预防术后吻合口瘘的效果及对术后恢复的影响

Effect of Anastomotic Reinforcement Suture in Preventing Postoperative Anastomotic Fistula during Laparoscopic Radical Resection of Middle and Low Rectal Cancer and Its Influence on Postoperative Recovery

目的:探讨腹腔镜下中低位直肠癌根治术中应用吻合口加固缝合操作的临床效果。方法:选取2022年11月—2023年11月于淄博市淄川区医院接受腹腔镜下中低位直肠癌根治术治疗的直肠癌患者60例为研究对象,以随机数字表法分为参照组与试验组,每组30例。参照组选择常规吻合形式,试验组选择吻合口加固缝合方式。对比两组临床相关指标、术后炎性指标、并发症发生情况。结果:两组术中出血量比较,无统计学差异(P>0.05);试验组手术时间长于参照组,首次排气时间、首次下床活动时间、胃肠功能恢复时间、住院时间短于参照组(P<0.05)。术后3 d,两组白细胞、中性粒细胞、单核细胞、淋巴细胞数目及C反应蛋白水平升高,试验组低于参照组(P<0.05)。试验组并发症总发生率低于参照组(P=0.037)。结论:腹腔镜下中低位直肠癌根治术中应用吻合口加固缝合操作的临床效果较好,虽手术时间较长,但可促进患者术后恢复,减轻机体炎性反应,降低并发症发生率。

Objective:To investigate the clinical effect of anastomotic reinforcement suture in laparoscopic radical resection of middle and low rectal cancer.Methods:Sixty patients with rectal cancer who received laparoscopic radical resection for middle and low rectal cancer in Zibo Zichuan District Hospital from November 2022 to November 2023 were selected as the study subjects,and were divided into reference group and experimental group by random number table method,with thirty cases in each group.The conventional anastomosis was selected in the reference group,and the anastomosis reinforcement suture was selected in the experi⁃mental group.Clinical indicators,postoperative inflammation indicators and complications were compared between the two groups.Results:There was no significant difference in intraoperative blood loss between the two groups(P>0.05);the operation time of experi⁃mental group was longer than that of reference group,and the first exhaust time,first getting out of bed activity time,gastrointestinal function recovery time and hospital stay time of experimental group were shorter than that of reference group(P<0.05);3 days after op⁃eration,the number of leukocytes,neutrophils,monocytes,lymphocytes and C-reactive protein levels in two groups were increased,and the experimental group was lower than the reference group(P<0.05);the total complication rate of experimental group was lower than that of reference group(P=0.037).Conclusion:The application of anastomotic reinforcement suture in laparoscopic radical re⁃section of middle and low rectal cancer has good clinical effect.Although the operation time is longer,it can promote the postopera⁃tive recovery of patients,reduce the inflammatory response of the body,and reduce the incidence of complications.
