络欣胶囊联合达格列净治疗糖尿病肾病的临床研究

Clinical study of Keluoxin capsule combined with dapagliflozin in the treatment of diabetic nephropathy

目的探讨渴络欣胶囊联合达格列净治疗糖尿病肾病的效果。方法选取2022年4月至2023年10月陕西省人民医院收治的90例糖尿病肾病患者,根据随机数字表法将患者分为对照组和观察组,每组45例。2组患者均接受常规对症支持治疗,对照组给予达格列净治疗,观察组在对照组基础上给予渴络欣胶囊治疗,2组均连续治疗3个月。比较2组患者临床疗效、治疗前后的胰岛功能指标、肾功能指标、血糖指标、炎症指标、不良反应发生情况。结果观察组总有效率高于对照组[88.9%(40/45)比71.1%(32/45)],差异有统计学意义(χ^(2)=4.395,P=0.036)。治疗后,观察组胰岛素抵抗指数、血肌酐、尿素氮、24 h尿蛋白定量、空腹血糖、餐后2 h血糖、糖化血红蛋白、高敏C反应蛋白、白细胞介素1β、白细胞介素6及肿瘤坏死因子α水平均小于/低于对照组,胰岛β细胞功能指数大于对照组,差异均有统计学意义(均P<0.05)。2组不良反应发生率比较差异无统计学意义(P=0.398)。结论渴络欣胶囊联合达格列净治疗糖尿病肾病的效果显著,可改善胰岛功能、肾功能,降低炎症水平,且安全性较好。

Objective To investigate the effect of Keluoxin capsule combined with dapagliflozin in the treatment of diabetic nephropathy.Methods A total of 90 patients with diabetic nephropathy admitted to Shaanxi Provincial People′s Hospital from April 2022 to October 2023 were selected.According to the random number table method,the patients were divided into control group and observation group,with 45 cases in each group.Patients in both groups received routine symptomatic and supportive treatment.The control group was treated with dapagliflozin,and the observation group was treated with Keluoxin capsule on the basis of the control group.Both groups were treated for 3 months.The clinical efficacy,islet function indexes,renal function indexes,blood glucose indexes,inflammatory indexes before and after treatment and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group[88.9%(40/45)vs 71.1%(32/45)](χ^(2)=4.395,P=0.036).After treatment,the insulin resistance index,serum creatinine,urea nitrogen,24 h urine protein,fasting blood glucose,postprandial 2 h blood glucose,glycosylated hemoglobin,high-sensitivity C-reactive protein,interleukin-1β,interleukin-6 and tumor necrosis factor-αlevels in the observation group were smaller than/lower than those in the control group,and the isletβcell function index was higher than that in the control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P=0.398).Conclusion Keluoxin capsule combined with dapagliflozin is effective in the treatment of diabetic nephropathy,which can improve islet function and renal function,reduce the level of inflammation,and has good safety.
