基于“天-人-病”理论探讨糖尿病视网膜病变与运气禀赋的相关性

Correlation Between Diabetic Retinopathy and Wu Yun Liu Qi and Endowment Based on"Heaven-Human-Disease"Theory

目的:探讨糖尿病视网膜病变(diabetic retinopathy,DR)与运气禀赋的相关性。方法:收集北京中医药大学东直门医院东城院区2017年1月20日至2022年1月20日诊断为2型糖尿病(diabetes mellitus type 2,T2DM)的病历,经过数据筛查,最终筛选出3512份病历,统计患者的一般资料及五运六气资料,通过单因素分析及多因素Logistic回归分析,探讨DR与一般临床资料及运气禀赋的相关性。结果:(1)χ^(2)分析结果:与单纯的T2DM患者相比较,DR患者主运为少土所占比例显著提高,主运为少金所占比例明显降低,差异具有统计学意义(P<0.05)(2)二分类Logistic回归分析结果:从岁运分析,水运不及的人群更容易罹患DR;从主运分析,少金的人群不易患DR;从司天之气分析:少阴君火司天和太阴湿土司天的人群更容易罹患DR;从在泉之气分析:阳明燥金在泉和太阳寒水在泉的人群更容易罹患DR;从主气客气分析,主气为阳明燥金,客气为太阴湿土的人群更容易罹患DR;从相得顺逆分析:主生客和主克客的人群更不容易罹患DR;从运气相合分析:运气相合为不和的人群更容易罹患DR,差异均具有统计学意义(P<0.05)。结论:T2DM患者的运气禀赋特征与DR有一定的相关性,DR患者多由于肝木、肾水受邪或脾土不足,脾虚无力散精,导致双目失养从而出现DR。

Objective:To explore the correlation between diabetic retinopathy(DR)and luck endowment.Methods:Medical records of patients diagnosed with type 2 diabetes mellitus type 2(T2DM)in Dongcheng Hospital branch of Dongzhimen Hospital Affiliated to Beijing University of Chinese Medicine from January 20,2017 to January 20,2022 were collected,and 3512 medical records were finally screened after data screening,and the general information of patients and the five lucks and six qi were counted to explore the correlation of DR with general clinical information and luck endowment through univariate analysis and multifactorial Logistic regression analysis was performed to explore the correlation between DR and general clinical information and Yun Qi and endowment.Results:(1)Results of chi-square analysis:compared with simple T2DM patients,the proportion of DR patients with less Earth in their main Yun was significantly higher,and the proportion of patients with less Gold in their main Yun was significantly lower,and the difference was statistically significant(P<0.05)(2)Results of two-category logistic regression analysis:from the analysis of yearly Yun,the people who had less Water Yun were more likely to develop DR;from the analysis of main Yun,the people who had less Gold are less likely to develop DR;From the perspective of the Qi of Sitian,those with Shaoyin Junhuo Sitian and Taiyin Shitu Titian were more likely to develop DR;from the perspective of the Qi of Zaiquan,those with Yangming Zaojin Zaiquan and those with Taiyang Hanshui Zaiquan were more likely to suffer from DR;from the perspective of Zhuqi and Keqi,those with Yangming Zaojin Zhuqi and those with Taiyin Shitu Keqi were more likely to suffer from DR;from the perspective of Shun or Ni condition of the Qi:those with Zhu generating the Ke and those with Zhu restrain the Ke were less likely to suffer from DR;from the perspective of Yun Qi relation:those with bad relationship in Yun and Qi were more likely to suffer from DR,and the differences were statistically significant(P<0.05).Conclusion:There is a certain correlation between the Yun Qi and endowment characteristics of T2DM patients and DR,and DR patients are more likely due to Liver Wood,Kidney Water injury by the evil factors or Spleen Earth deficiency,and the inability in dispersing Essence due to Spleen deficiency leads to the loss of nourishment of the eyes and thus the emergence of DR.
