摘要
构建营养不良的临床预测模型,以期为消化道恶性肿瘤患者的临床营养支持与干预治疗提供更加有效的风险预测指标及模型。回顾性研究2022年3月—2022年12月在中山大学附属第六医院胃外科二区住院的147例择期手术患者,采用患者主观整体营养评估表(PG-SGA)评估术前患者的营养状态,应用二分类Logistic回归分析营养不良的独立危险因素,采用R软件构建列线图预测模型。多因素Logistic分析显示,BMI、上臂围度、小腿围、结肠癌、胃食管癌是消化道恶性肿瘤患者中重度营养不良的影响因素(P<0.05)。研究发现,基于BMI、上臂围度、小腿围和消化道肿瘤部位构建的临床预测模型对消化道恶性肿瘤患者术前营养不良状况具有较好的预测作用,若模型预测患者营养不良的发生风险较高,建议进行预防性护理干预。
Constructing a clinical prediction model for malnutrition,in order to provide more effective risk prediction indicators and models for clinical nutritional support and intervention treatment of patients with gastrointestinal malignancies.A retrospective study was conducted on 147 elective surgical patients admitted to the Department of Gastroenterology,Second District,The Sixth Affiliated Hospital of Sun Yat-sen University from March 2022 to December 2022.The subjective global nutritional assessment(PG-SGA)was used to evaluate the preoperative nutritional status of patients,and binary logistic regression was applied to analyze independent risk factors for malnutrition.A column chart prediction model was constructed using R software.Multivariate logistic analysis showed that BMI,upper arm circumference,lower leg circumference,colon cancer,and gastrointestinal cancer were the influencing factors of moderate to severe malnutrition in patients with gastrointestinal malignancies(P<0.05).Research has found that clinical prediction models based on BMI,upper arm circumference,lower leg circumference,and location of gastrointestinal tumors have a good predictive effect on preoperative malnutrition in patients with gastrointestinal malignancies.If the model predicts a high risk of malnutrition in patients,preventive nursing interventions are recommended.
